The intel x4x and gm45 have very similar integrated graphic devices.
Currently the x4x native graphic init enables VGA, while gm45 can output
on LVDS.
This patch reuses the x4x graphic initialisation code
to enable output on VGA in gm45 in a way that the behavior is similar to vbios:
If no VGA display is connected the internal LVDS screen is used.
If an external screen is detected on the VGA port it will be used instead.

Replace the use of the old device_t definition inside
northbridge/intel/gm45.
The patch has been tested both with the arch/io.h definition of device_t
enabled and disabled in order to ensure compatibility while the
transaction takes place.

Previously the ME PCI interface (HECI) was being reported as present in
the DMAR ACPI table even when ME firmware was missing or the PCI device
was hidden and HECI would be unresponsive.
Now we check via the PCI config space itself to verify if the HECI
is present or not.
Note that this test could fail if ME firmware is present but
HECI is disabled in devicetree, because it would not advertise that the HECI
exists even though there is a running ME. Perhaps this behaviour is desirable
because in this case you won't see the HECI in the lspci tree anyway.

According to "G45: Volume 3: Display Register
Intel ® 965G Express Chipset Family and Intel ®
G35 Express Chipset Graphics Controller" some registries
are set incorrectly in gm45/gma.c.
Some values are changed after comparing them with the values
the i915 linux kernel (3.13 was used) module sets while modesetting.
The values were obtained using 'intel_reg' from intel-gpu-tools,
during a normal boot and with 'nomodeset' as a kernel argument.
Some registers that don't exist on gm45 are set in gma.c, which is
probably the result of copying code from a more recent intel
northbridge.
The result is that that gm45 laptops with wxga displays still work as
before. gm45 laptops with wxga+ or higher resolution now just work,
where previously a black screen was shown.
TEST: build with native graphic init and flash on a gm45 target, like
lenovo x200.

Instead of hardcoding the PCI mmio size read it from devicetree.
Set a default value of 2048 MiB and 1024MiB for laptops without
discrete graphics.
Tested on Sandybridge Lenovo T520.

Setting the size of the preallocated memory for the igd is done
using a cmos parameter, gfx_uma_size. This was limited to a subset of
all available sizes, that were already implemented elsewhere
in the northbridge code.
What this does is change the cmos parameter to 4 bits instead
of 3 bits to accomodate all vram sizes.
It also adds a sane default of 32mb that already was in place.
The northbridge code that reads this cmos parameter is
also changed for this new cmos settings.
352M is disabled since it causes issues on systems with 4GB or more ram.
TEST: Build, flash target. Clear cmos by corrupting
the checksum (nvramtool -c something).
Set a desired value in gfx_uma_size using nvramtool.
"dmesg | grep stolen" to see what is actually allocated.

The LVDS port is configured to accept data from pipe A, but the panel
fitter and VGA were attached to pipe B.
Changes to VGACNTRL:
- select pipe A instead of pipe B.
- disable VGA centering to fix jitter.
TEST=Build and run on Thinkpad X200 in both text and framebuffer modes.

We want the question for CBFS size to be next to the rom size in the
mainboard directory, but that doesn't seem to work for how people
want to set the defaults. Instead of having the list of exceptions
to the size, just set the defaults at the end of kconfig.
- Move the defaults for chipsets not setting HAVE_INTEL_FIRMWARE into
the chipset Kconfigs (gm45, nehalem, sandybridge, x4x)
- Override the default for HAVE_INTEL_FIRMWARE on skylake.
- Move the HAVE_INTEL_FIRMWARE default setting into the firmware
Kconfig file
- Move the location of the default CBFS_SIZE=ROM_SIZE to the end of
the top level kconfig file, while leaving the question where it is.
Test=rebuild Kconfig files before and after the change, verify that
they are how they were intended to be.
Note: the Skylake boards actually changed value, because they were
picking up the 0x100000 from HAVE_INTEL_FIRMWARE instead of the
0x200000 desired. This was due to the SOC_INTEL_SKYLAKE being after
the HAVE_INTEL_FIRMWARE default. Affected boards were:
Google chell, glados, & lars and Intel kunimitsu.

This reverts commit 0e06f5bd70b45fd330d8dfb1dc77cce043caf841.
It breaks gm45 and also does some magic without being asked too. It
disables bridge devices permanently if no device was found on the se-
condary bus. In a simple notebook world this might be ok, but it breaks
hot-plugging and late detection (if a secondary bus device comes up too
slow for the firmware to detect and the OS has to enumerate it).

Skip everything but the final setting of PP_CONTROL, i.e. triggering
the power up. The settings with PANEL_UNLOCK_REGS are useless as no
lockable registers were touched in between. Also the loop waiting for
the panel power up to finish was a no-op as the registers with the
power timings were never filled (see follow-up commits).

Make the low-power and small form factor (SFF) options overridable
from romstage main. Also disable both options by default. That's ok
as there aren't yet any in-tree users of the GS45 chipset. As a nice
side-effect, this adds X200s support to the lenovo/x200 port.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

On Broadwell, this reduces the number of 'remarks' in the IASL build
from 222 to 3.
Fixes these remarks:
Object is not referenced (Name is within method [_CRS])
The ACPI compiler is trying to be helpful in letting us know
that we're not using various fields in the MCRS ResourceTemplate
when we define it inside of the _CRS method. Since we're not
intending to use those objects in the method, it shouldn't be an
issue, but the warning is annoying and can mask real issues.
Moving the creation of the MCRS object to outside of the CRS
method and referencing it from there solves this problem.

Consolidate some common (and mostly broken) code. Will try to fix things
in separate commits.
Maybe, igd.asl taken from gm45 (the non-PCH case) could also be used for
i945 and sch. But this needs further investigation.

Only one value would work with corresponding gma code currently (which one
depends on board). Going forward, it's possible to compute which number can
be used, so there is no need to keep this info around.

In the gm45 code, IOMMU is always selected to be enabled. Instead
this patch removes the Kconfig symbol and its dependencies. This leads
to the same effect without the need for the symbol.
The symbol is still used in the K8 code as it's not selected, simply
defaulted to being enabled, and one of the mainboards disables it.

The CBFS size is really mainboard specific, since it really depends on
size of the chip on the mainboard, so it makes sense to have it in
the mainboard menu along with the ROM-chip size.
- Move the CBFS_SIZE definition up in src/kconfig
- Move the Mainboard Menu markers out of src/mainboard/kconfig into
src/Kconfig so CBFS_SIZE can live in the mainboard menu.
- Add a long list setting default values to do what the chipset
directories were previously defaulting the values to. This will
be trimmed down in a following patch that creates a common set of
IFD routines. (Who knew that kconfig supported line wrapping?)
- Update the help text.

Old igd.asl had inconsistent addresses (between _DOD and actual device)
and ghost devices. Any of those is enough to make brightness on windows
fail and make igd.asl out-of-ACPI-spec. Also old code favoured ridiculous
copying of the same thing 6 times per chipset. Leave only hooking up and
chipset-specific part in chipset directory. Move NVS handling and ACPI-spec
parts to a common file.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

This change switches all northbridge vendors and southbridges
to be autoincluded by Makefile.inc, rather than having to be
mentioned explicitly in northbridge/Makefile.inc or in
northbridge/<vendor>/Makefile.inc.
This means, vendor and northbridge directories are now "drop
in", e.g. be placed in the coreboot directory hierarchy
without having to modify any higher level coreboot files.
The long term plan is to enable out of tree components to be
built with a given coreboot version (given that the API did not
change).

On x86, change the type of the address parameter in

Drop the implementation of statically allocated high memory
region for CBMEM. There is no longer the need to explicitly

The name was always obscure and confusing. Instead define cbmem_top()
directly in the chipset code for x86 like on ARMs.

There were instances of unneeded arch/hlt.h includes,
various hlt() calls that weren't supposed to exit (but
might have) and various forms of endless loops around
hlt() calls.
All these are sorted out now: unnecessary includes are
dropped, hlt() is uniformly replaced with halt() (except
in assembly, obviously).

The tests for __PRE_RAM__ or __SMM__ were repeatedly used
for detection if dev->ops in the devicetree are not available
and simple device model functions need be used.
If a source file build for ramstage had __PRE_RAM__ inserted
at the beginning, the struct device would no longer match the
allocation the object had taken. This problem is fixed by
replacing such cases with explicit __SIMPLE_DEVICE__.

Define at one place whether to use IO 0xcf8/0xcfc or MMIO via
MMCONF_BASE_ADDRESS for PCI configuration access funtions in ramstage.
The implementation of pci_default_config() always returned with
pci_cf8_conf1. This means any PCI configuration access that did
not target bus 0 used PCI IO config operations, if PCI MMIO config
was not explicitly requested.

I missed the board with gm45 when I moved MMCONF_SUPPORT lines.
Also, the intel/i3100 does not have MMCONF_SUPPORT implemented
even though it was previously selected for intel/eagleheights board.

multiply_to_tsc was being copied everywhere, which is bad
practice. Put it in the tsc.h include file where it belongs.
Delete the copies of it.
Per secunet, no copyright notice is needed.
This might be a good time to get a copyright notice into tsc.h
anyway.

When configuring the GTT size for the integrated graphics, the state
of VT-d was read wrong. Bit 48 of CAPID0 (D0F0) is set when VT-d is
_disabled_.
In the log of a VT-d enabled roda/rk9 we have now:

VT-d enabled
[...]
IGD decoded, subtracting 32M UMA and 4M GTT
[...]
Without this patch, only 2M GTT were reported.

Here's the great news: From now on you don't have to worry about
hitting the right io.h include anymore. Just forget about romcc_io.h
and use io.h instead. This cleanup has a number of advantages, like
you don't have to guard device/ includes for SMM and pre RAM
anymore. This allows to get rid of a number of ifdefs and will
generally make the code more readable and understandable.
Potentially in the future some of the code in the io.h __PRE_RAM__
path should move to device.h or other device/ includes instead,
but that's another incremental change.

It's been on for all boards per default since several years now
and the old code path probably doesn't even work anymore. Let's
just have one consistent way of doing things.

The name lapic_cluster is a bit misleading, since the construct is not local
APIC specific by concept. As implementations and hardware change, be more
generic about our naming. This will allow us to support non-x86 systems without
adding new keywords.

The name pci_domain was a bit misleading, since the construct is only
PCI specific in a particular (northbridge/cpu) implementation, but not
by concept. As implementations and hardware change, be more generic
about our naming. This will allow us to support non-PCI systems without
adding new keywords.

As we move to supporting other systems we need to get rid of assembly
where we can. The log2 function in src/lib is identical to the assembly
one (tested for all 32-bit signed integers :-) and takes about 10 ns
to run as opposed to 5ns for the non-portable assembly version. While speed
is good, I think we can spare the 15 ns or so we add to boot time
by using the C version only.

The code supports DDR3 boards only. RAM init for DDR2 is sufficiently
different that it requires separate code, and we have no boards to
test that.
