Looks like PCIe root port device IDs for 9-series PCH-H are missing from
commit 434d7d45829e (sb/intel/lynxpoint: Add PCI DIDs for 9 series PCHs)
for some reason. Add them, so that coreboot performs PCIe initialisation
for 9-series PCH-H.

Program the AER capability header register in a single write because
it's write-once. In addition, only PCH-LP supports L1 sub-states, so
only report the L1 sub-state capability on PCH-LP. This follows what
Lynx Point PCH reference code version 1.9.1 does.

Lynx Point PCH reference code version 1.9.1 programs the larger L1 exit
latency when ASPM is enabled. Document 535127 (BDW PCH-LP BS) also does
the same. Correct the condition accordingly. On Lynx Point, also remove
a now-redundant write to the LCAP register (offset 0x4c).

Lynx Point PCH reference code version 1.9.1 checks bit 29 to detect ASPM
on PCH-LP root port #6, not bit 28. Document 535127 (BDW PCH-LP BS) also
uses bit 29 for root port #6. Correct the bit used in the check, as well
as the surrounding comments.

Shorten define names containing PCI_{DEVICE,VENDOR}_ID_ with
PCI_{DID,VID}_ using the commands below, which also take care of some
spacing issues. An additional clean up of pci_ids.h is done in
CB:61531.

Setting registers 64h[19:18] = 2 and 68h[14:13] = 3 enables OBFF, and
setting registers 64h[19:18] = 0 and 68h[14:13] = 0 disables OBFF.
Register at offset 0x64 is DCAP2, and offset 0x68 is DCTL2.
However, current code doesn't account for this. The result is that
register 64h[19:18] = 2 and 68h[14:13] = 0, which means the hardware is
OBFF-capable but support is disabled, which makes no sense. Given that
reference code and Broadwell both disable OBFF, disable it here too.

Several changes[1][2] to the Linux kernel now enable ASPM/AER for the
rt8169 network driver, for which it was previously disabled. This,
coupled with the southbridge enabling AER for all PCIe devices, has
resulted in a large amount of AER timeout errors in the kernel log for
boards which utilize the rt8169 for on-board Ethernet (e.g., google/beltino).
While performance is not impacted, the errors do accumulate.
To mitigate this, guard AER enablement via Kconfig, select it by default
(as to maintain current default behavior), and allow boards which need
to disable it to do so (implemented in subsequent commits).
This implementation is derived from that in soc/intel/broadwell.
Test: build/boot google/beltino variants with AER disabled (CB:46136),
verify dmesg log free of AER timeout errors.

Some cases could not be factored out while keeping reproducibility.
Also mark some potential bugs with a FIXME comment, since fixing them
while also keeping the binary unchanged is pretty much impossible.

PCIe RPC (Root Port Configuration) straps will force-disable some root
port functions if some root ports have a width greater than x1. In two
cases, this affects the last function. The PCIe init code will never
finish configuring the root ports if that is the case: it assumes that
the last function will eventually run through the code, but it doesn't.
If PCIe initialization does not complete, pressing the power button will
not power off the board, unless it is held for about five seconds. Also,
Windows 10 will show a BSOD about MACHINE CHECK EXCEPTION, and lock up
instead of rebooting. Depending on the microcode version, the BSOD may
not be visible. This happens even when the root port is not populated.
Use the strap fuse configuration value to know which configuration the
PCH is strapped to. If needed, update the number of ports accordingly.
In addition, print the updated value to ease debugging PCIe init code.
Existing code in coreboot disagrees with public documentation about the
root port width straps. Assume existing code is correct and document
these assumptions in a table, as an explanation for the added code.
Tested on Asrock B85M Pro4, PCIe initialization completes successfully.

Looks like no one really knows what this bit would be useful for, nor
when it would need to be set. Especially if coreboot is setting it even
on PCI *Express* bridges. Digging through git history, nearly all
instances of setting it on PCIe bridges comes from i82801gx, for which
no reason was given as to why this would be needed. The other instances
in Intel code seem to have been, unsurprisingly, copy-pasted.
Drop all uses of this definition and rename it to avoid confusion. The
negation in the name could trick people into setting this bit again.
Tested on Asrock B85M Pro4, no visible difference.

They're listed in AUTHORS and often incorrect anyway, for example:
- What's a "Copyright $year-present"?
- Which incarnation of Google (Inc, LLC, ...) is the current
copyright holder?
- People sometimes have their editor auto-add themselves to files even
though they only deleted stuff
- Or they let the editor automatically update the copyright year,
because why not?
- Who is the copyright holder "The coreboot project Authors"?
- Or "Generated Code"?
Sidestep all these issues by simply not putting these notices in
individual files, let's list all copyright holders in AUTHORS instead
and use the git history to deal with the rest.

If PCIe root port `n` is disabled, then `rpc.ports[n - 1]` remains NULL.
The existing Lynx Point systems probably don't end up dereferencing
NULL pointers this way. However, it might occur on a system using
Flexible I/O to remap PCIe root ports to other functions.
Tested on an ASRock H81M-HDS and an Acer C720 (Google Peppy). No issues
presented themselves.

The H81 chipset is the only non-LP Lynx Point chipset with 6 PCIe root
ports, all others have 8 [1]. The existing PCIe code assumed that all
non-LP chipsets had 8 root ports, which meant that port 6 would not be
considered the last root port on H81, so `root_port_commit_config()`
would not run. Ultimately, while PCIe still worked on H81, all the root
ports would remain enabled, even if disabled in the devicetree.
Also, remove `PCI_DEVICE_ID_INTEL_LYNXPOINT_MOB_DESK_{MIN,MAX}`, as they
are unused, and the MAX constant is incorrect.
Interestingly, this fixes an issue where GRUB is unable to halt the
system.
Tested on an ASRock H81M-HDS. The root ports disabled in the devicetree
do indeed end up disabled.

>= the width of the type is undefined. Add UL suffix since it's safe
for unsigned types.

INTEL_LYNXPOINT_LP, which has it's own gpio code.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

The PCIe root port has ASPM settings/workarounds that are only applied
based on the value of an undocumented bit in PCI config register 0x32C.
If that bit is not set for some reason then the settings are not applied.
This devicetree config option will force the ASPM settings for each port
based on the bit map.

The BIOS spec for LynxPoint calls out additional
programming steps for the PCIe Root Ports. Implement those
steps from the BIOS spec. These steps are completed before
deeper PCIe probing. The "late" programming was removed as
that was applicable to Cougar/Panther point where this
code was originally copied, though there was some overlap.

PCIe Root Ports should be disabled based on pin ownership
and the strapping configuration. Implement this logic
for LynxPoint. The chip_ops->enable_dev() path is no
longer used. Instead the PCIe driver handles the enabling
and disabling of devices. This allows for having an empty
or incomplete device tree since those "allocated" devices
do not travel through the chip_ops->enable_dev() path.
The coalescing was tested to be working properly, however
not all configurations were tested.

Some of the pcie logic was located in pch.c as well
as pcie.c. Move all pcie logic to the same pcie.c
file. This is a straight cut-and-paste (no logic changes)
except for a rename from pch_pcie_enable() ->
pch_pcie_enable_dev().

The Haswell parts use a PCH code named Lynx Point (Series 8). Therefore,
the southbridge support is included as well. The basis for this code is
the Sandybridge code. Management Engine, IRQ routing, and ACPI still requires
more attention, but this is a good starting point.
This code partially gets up through the romstage just before training
memory on a Haswell reference board.
