This allows dropping some preprocessor usage. The `mkhi_end_of_post`
static functions had to be renamed to avoid a name clash. A follow-up
will tidy up the code in me_smm.c to reduce some duplication.

Having some symmetry with <soc/nvs.h> now allows to reduce
the amount of gluelogic to determine the size and cbmc field
of struct global_nvs.
Since GNVS creation is now controlled by ACPI_SOC_NVS,
drivers/amd/agesa/nvs.c becomes obsolete and soc/amd/cezanne
cannot have this selected until <soc/nvs.h> exists.

They're listed in AUTHORS and often incorrect anyway, for example:
- What's a "Copyright $year-present"?
- Which incarnation of Google (Inc, LLC, ...) is the current
copyright holder?
- People sometimes have their editor auto-add themselves to files even
though they only deleted stuff
- Or they let the editor automatically update the copyright year,
because why not?
- Who is the copyright holder "The coreboot project Authors"?
- Or "Generated Code"?
Sidestep all these issues by simply not putting these notices in
individual files, let's list all copyright holders in AUTHORS instead
and use the git history to deal with the rest.

There is some overlap between romstage and bootblock.
LPC setup and BAR initialization is now done twice.
The rationale is that the romstage should not depend too
much on the bootblock, since it can reside in a RO fmap
region.
Enabling the console will be done in a followup patch.

Commit 45d4b17 [nb/intel/sandybridge: Move southbridge code to bd82x6x]
moved early_pch_init() to the southbridge, but failed to include
early_pch.c for the non-native raminit case, which now fails to link.
As all boards default to native raminit, this was missed by the autobuilder.
Adjust early_pch.c to be compiled regardles of ram init type used

The common finalise code is used by bd82x6x, Lynx Point, and Ibex Peak.
Lynx Point now benefits from being able to write-protect the flash chip.
For Lynx Point, writing the SPI OPMENU now happens in ramstage, as done
in bd82x6x.
Tested on an ASRock H81M-HDS (Lynx Point). When write-protection is
configured, flashrom reports all flash regions as read-only, and does
not manage to alter the contents of the flash chip.
Also tested on an ASUS P8H61-M LX (Cougar Point). Everything seems to
work as before.

This is a sad story. We have three different code paths for
sandybridge and ivybridge: proper native path, google MRC path, and,
everyone's favorite: Intel FSP path. For the purpose of this patch,
the FSP path lives in its own little world, and doesn't concern us.
Since MRC was first, when native files and variables were added, they
were suffixed with "_native" to separate them from the existing code.
This can cause confusion, as the suffix might make the native files
seem parasitical.
This has been bothering me for many months. MRC should be the
parasitical path, especially since we fully support native init, and
it works more reliably, on a wider range of hardware. There have been
a few board ports that never made it to coreboot.org because MRC would
hang.
gigabyte/ga-b75m-d3h is a prime example: it did not work with MRC, so
the effort was abandoned at first. Once the native path became
available, the effort was restarted and the board is now supported.
In honor of the hackers and pioneers who made the native code
possible, rename things so that their effort is the first class
citizen.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

This change switches all southbridge vendors and southbridges
to be autoincluded by Makefile.inc, rather than having to be
mentioned explicitly in southbridge/Makefile.inc or in
southbridge/<vendor>/Makefile.inc.
In order to be able to drop southbridge/amd/Makefile.inc, some
scattered source files had to be moved to a southbridge/amd/common
directory, in accordance to what we are doing on other architectures
already.
This means, vendor and southbridge directories are now "drop
in", e.g. be placed in the coreboot directory hierarchy
without having to modify any higher level coreboot files.
The long term plan is to enable out of tree components to be
built with a given coreboot version (given that the API did not
change).

On those chipsets the pins are just a legacy concept. Real interrupts are
messages on corresponding busses or some internal logic of chipset.
Hence interrupt routing isn't anymore board-specific (dependent on layout) but
depends only on configuration.
Rather than attempting to sync real config, ACPI and legacy descriptors, just
use the same interrupt routing per chipset covering all possible devices.
The only part which remains board-specific are LPC and PCI interrupts.
Interrupt balancing may suffer from such merge but:
a) Doesn't seem to be the case of this map on current systems
b) Almost all OS use MSI nowadays bypassing this stuff completely
c) If we want a good balancing we need to take into account that e.g.
wlan card may be placed in a different slot and so would require complicated
balancing on runtime. It's difficult to maintain with almost no benefit.

Current build configuration always wants to include an Intel Management
Engine firmware (me.bin) on Sandy Bridge systems. However, we can have
a working coreboot without it, as long as the factory delivered ME
firmware is kept untouched in the flash ROM. So let the user decide if
a ME firmware will be included in the build.

On newer Intel systems, the flash ROM is shared between the host
processor (BIOS), it's Management Engine (ME) and an integrated ethernet
controller (GbE). The layout of the flash ROM (and other information) is
kept in the so called Intel Firmware Descriptor (IFD). If we only want
to build coreboot to update the BIOS section, all we need is the flash
layout.
This patch adds the option to specify the flash layout in the
mainboard's Kconfig, and thus, to build without the real IFD. However,
with such a build, one has to make sure that the IFD section on the
flash ROM won't be written over (nor any other section that hasn't been
included by coreboot). A patch to write selected sections of a flash ROM
with IFD has been sent to the flashrom mailing list [1].

There is no policy on how to order these additions, so leave
it to a possible separate commit, unifying this.

At some point, compiles with USB Debug port stopped working. This change makes
a trivial reordering in the code and adds two makefile entries to make it build
without errors. It also works on stout.
Build and boot as normal. Works. Enable CONFIG_USB, connect USB debug hardware
to the correct port (on stout, that's the one on the left nearest the back) and
watch for output.

The use of ramstage.a required the build system to handle some
object files in a special way, which were put in the drivers
class.
These object files didn't provide any symbols that were used
directly (but only via linker magic), and so the linker never
considered them for inclusion.
With ramstage.a gone, we can drop this special class, too.

Right now coreboot's build process produces images that are
not booting on actual hardware because they are smaller than
the actual flash device and also don't have an IFD nor an ME
firmware in them. In order to produce bootable images, you
needed a wrapper script / extra step until now. With this
change, the resulting coreboot.rom is actually bootable.

- make flash chip structure non-const so the probe function
pointers can be relocated for use in TSEG
- Make SMM PCI access possible in southbridge SPI code

In the short term there might be devices with Sandy Bridge CPUs
on mainboards with Panther Point PCHes. While this configuration
option is perfectly valid, coreboot currently ties Sandy Bridge to
Cougar Point and Ivy Bridge to Panther Point. One occurence is in
the ME handling code.
To make coreboot most flexible, compile both ME handlers into
coreboot and decide at runtime which one to use.
