The usage of `pci_devfn_t` here is misleading, as these intentionally
store the `PCH_DEVFN_*` macros so they can be used across `smm` and
`ramstage` without requiring the device model. Update to `unsigned int`
instead, as `pci_devfn_t` implies the data is an MMCONF-compatible PCI
devfn offset.

Currently this XHCI driver assumes the PCH XHCI controller, but the TCSS
or North XHCI block has a similar enough PCI MMIO structure to make this
code mostly reusable.
1) Rename everything to drop the `pch_` prefix
2) xhci_update_wake_event() now takes in a pci_devfn_t for the XHCI
controller
3) soc_get_xhci_usb_info() also now takes a pci_devfn_t for the XHCI
controller

They're listed in AUTHORS and often incorrect anyway, for example:
- What's a "Copyright $year-present"?
- Which incarnation of Google (Inc, LLC, ...) is the current
copyright holder?
- People sometimes have their editor auto-add themselves to files even
though they only deleted stuff
- Or they let the editor automatically update the copyright year,
because why not?
- Who is the copyright holder "The coreboot project Authors"?
- Or "Generated Code"?
Sidestep all these issues by simply not putting these notices in
individual files, let's list all copyright holders in AUTHORS instead
and use the git history to deal with the rest.

The current elog implemetation searches for an active PME status bit by
iterating the PCI devices. On disabled or hidden devices a BUG gets
triggered: BUG: pch_log_rp_wake_source requests hidden ...
This is caused by the use of the PCH_DEV_* macros which resolve to
_PCH_DEV and finally call pcidev_path_on_root_debug.
Disabled devices are skipped already so we can safely use the DEVFNs
instead, circumventing the BUG.

If GPE_STS indicates that the wake source is internal PME, but none of
the controllers have the PME_STS bit set, then try probing individual
XHCI ports to see if one of those was a wake source. In some cases
e.g. gsmi logging with S0ix, pci_pm_resume_noirq runs before gsmi
callback and clears PME_STS_BIT in controller register. In such cases,
xhci port status might provide a better idea about the wake source.

If Deep Sx is enabled the event log will get entries added on every
power sequence transition indicating that the suspend well has failed.
When a board is using Deep Sx by design this is intended behavior and
just fills the logs with extraneous events.
To make this work the device init state has to be executed first so it
actually enables the Deep Sx policies in the SOC since this code does
not have any hooks back into the devicetree to read the intended setting
from there.

Skip logging a wake source when just resetting without coming from
S3 or S5 state. This will prevent the occasional spurious event
like PCI PME from showing up in the event log.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

Some of the field and register names in the power management
code were not reflecting current chipset documentation. While
in there fix 0-sized array in the power_state structure. Lastly,
log the entire STD GPE register for visibility in elog. It reports
as an extension of other GPIO wake events.
