Currently this XHCI driver assumes the PCH XHCI controller, but the TCSS
or North XHCI block has a similar enough PCI MMIO structure to make this
code mostly reusable.
1) Rename everything to drop the `pch_` prefix
2) xhci_update_wake_event() now takes in a pci_devfn_t for the XHCI
controller
3) soc_get_xhci_usb_info() also now takes a pci_devfn_t for the XHCI
controller

TEST=plug in USB keyboard while in S0, enter S0ix and verify entry via
EC; type on keyboard, verify it wakes up, eventlog contains:

They're listed in AUTHORS and often incorrect anyway, for example:
- What's a "Copyright $year-present"?
- Which incarnation of Google (Inc, LLC, ...) is the current
copyright holder?
- People sometimes have their editor auto-add themselves to files even
though they only deleted stuff
- Or they let the editor automatically update the copyright year,
because why not?
- Who is the copyright holder "The coreboot project Authors"?
- Or "Generated Code"?
Sidestep all these issues by simply not putting these notices in
individual files, let's list all copyright holders in AUTHORS instead
and use the git history to deal with the rest.

Add API to disable USB devices that are not present but are configured
in the device tree either after probing the concerned port status or as
explicitly configured by the variants.

It feels appropriate to define SoC specific XHCI USB info in SoC
specific XHCI source file and an API to get that information instead of
defining it in elog source file. This will help in other situations
where the information is required.

The policy to identify and log the XHCI wake events is similar between
skylake and apollolake. Hence move the similar parts to a common
location.
