SPD decoding problems are no longer a good method for detecting if i2c

This simplifies computing dram timings a lot.
This removes computation of rank size based on columns, rows,
banks,... and uses the information in SPD byte 31. The result of this
is that dimms with multiple asymmetric ranks are not supported
anymore. These however are very rare and most likely never tested on
this platform.
This also uses i2c block read instead of byte read to speed up the
raminit. The result is less time is being spend reading SPDs.
It still keeps smbus read byte as a backup if i2c block read were to
fail.

Inspired by gm45 code, which sets this value the same way.
Some values for tRD on 800 and 1067MHz FSB were set wrong because the
CAS/Freq selection was wrong. CAS was often selected to low and when
fixing CAS this results in tRD being too high, due to an incorrect
lookup table which caused instability.
PASSED memtest86+ during 10h+ on 1067MHZ fsb with 667MHz ddr2, CAS 5

Vendor BIOS leaves UPMC1 untouched (on 945gc the default is 0x0203).
Not running PCIEx16 init which is valid for 945gm seems to fix all
issues and instabilities related to the PEG port.
According to lspci the link width is at the desired x16.
It is unknown if devices requesting a lower width work automatically
or need more configuration.
What happens is that IGD gets disabled by the disable function in
gma.c when an external GPU is found unless
CONFIG_ONBOARD_VGA_IS_PRIMARY is set.
Setting IGD as secondary makes Linux (4.10) hang, so this behavior is
a requirement for now.
TESTED on P5GC-MX with a discrete GPU and both
CONFIG_ONBOARD_VGA_IS_PRIMARY set and unset.

The code to set the igd frequencies is written with the mobile version
of the 945 chipset in mind and seems to cause cause strange igd
related problems on the desktop versions.
Some possible problems are:
* on 800MHz fsb CPUs the igd sometimes has artifacts on the screen;
* on 800MHz fsb CPU memtest results vary a lot;
* since a commit 45e11aa0a5 "Add/Combine Broadwell Chromebooks using
variant board scheme" that does not affect this northbridge, the
display shows garbage as soon as Linux (4.8) modesets the display.
A fix is to hardcode the core display and render clocks to their
maximum, potentially also improving graphical performance.
Vendor bios on all boards in coreboot with this northbridge have the
same value in this PCI config address.
TESTED on P5GC-MX (display works fine again in Linux) and
user reports of it making GA-945GCM-S2L run more stable.

The result is that 1067MHz FSB CPUs now boot without problems.

This patch adds a few bit counting functions that are commonly needed
for certain register calculations. We previously had a log2()
implementation already, but it was awkwardly split between some C code
that's only available in ramstage and an optimized x86-specific
implementation in pre-RAM that prevented other archs from pulling it
into earlier stages.
Using __builtin_clz() as the baseline allows GCC to inline optimized
assembly for most archs (including CLZ on ARM/ARM64 and BSR on x86), and
to perform constant-folding if possible. What was previously named log2f
on pre-RAM x86 is now ffs, since that's the standard name for that
operation and I honestly don't have the slightest idea how it could've
ever ended up being called log2f (which in POSIX is 'binary(2) LOGarithm
with Float result, whereas the Find First Set operation has no direct
correlation to logarithms that I know of). Make ffs result 0-based
instead of the POSIX standard's 1-based since that is consistent with
clz, log2 and the former log2f, and generally closer to what you want
for most applications (a value that can directly be used as a shift to
reach the found bit). Call it __ffs() instead of ffs() to avoid problems
when importing code, since that's what Linux uses for the 0-based
operation.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

On x86, log2() is defined as an inline function in arch/io.h. This is
a remnant of ROMCC, and forced us to not include clog2.c in romstage.
As a result, romstage on ARM has no log2().
Use the inline log2 only with ROMCC, but otherwise, use the one in
clog2.c.

Here's the great news: From now on you don't have to worry about
hitting the right io.h include anymore. Just forget about romcc_io.h
and use io.h instead. This cleanup has a number of advantages, like
you don't have to guard device/ includes for SMM and pre RAM
anymore. This allows to get rid of a number of ifdefs and will
generally make the code more readable and understandable.
Potentially in the future some of the code in the io.h __PRE_RAM__
path should move to device.h or other device/ includes instead,
but that's another incremental change.

Originally brought up by Sven Schnelle in March 2011
http://patchwork.coreboot.org/patch/2801/
http://www.coreboot.org/pipermail/coreboot/2011-March/064277.html
On some mainboards it may be neccessary to reset early during resume
from S3 if the SLFRCS register indicates that a memory channel is not
guaranteed to be in self-refresh.
On other mainboards, such as Lenovo X60 and T60, the check always
creates false positives, effectively making it impossible to resume.
The SLFRCS register is documented on page 197 of
Mobile Intel® 945 Express Chipset Family Datasheet
Document Number: 309219-006
which is publically available, and the register indicates if a memory
channel is guaranteed to be in self-refresh mode (if bit = 1), or that
a memory channel *may or may not be* in self-refresh mode (if bit = 0).
The register can thus only be used to positively learn that memory is
in self-refresh. It is not known for sure that memory is *not* in
self-refresh. The register is reset by the PWROK signal, which *should*
go low during S3, and go high again when resuming, so it is unsurprising
that SLFRCS has already been cleared when we read the register.
Sven's measurements of the CKE signal on a ThinkPad shows that memory
remains in self-refresh indefinitely, until coreboot re-initializes the
memory controller, even when SLFRCS bits were = 0.
Boards which require a warm reset when SLFRCS bits are cleared must now
explicitly enable the check in the mainboard Kconfig file.
This commit selects the new option in all existing i945 mainboards.
A follow-up commit will remove the option for ThinkPads.

The current code works only with dual channel if Channel 0 uses SPD address
0x50/0x51, while the second channel has to use 0x52/0x53.
For hardware that uses other addresses (like the ThinkPad X60) this means we
get only one module running instead of both.
This patch adds a second parameter to sdram_initialize, which is an array with
2 * DIMM_SOCKETS members. It should contain the SPD addresses for every single
DIMM socket. If NULL is given as the second parameter, the code uses the old
addressing scheme.

3 (with one of them way too much assembler code).
On the way, I had to make some changes to the way the code is built,
which is an effort I want to expand over time.
Right now, large portions of the in-ROM part of coreboot is compiled as
a single file, with lots of .c files including other .c files.
That has its justification for pre-raminit code, but it also affects
lots of post-raminit code (memcpy doesn't really make sense before
raminit, or at least CAR)
The coreboot_apc code (AMD boards) gained some .c includes because I
don't know that part of the code enough to really rework it and only
have limited possibilities to test it. The includes should give an
identical situation for this part of the code.
This change was posted as set of 6 patches to the list, but they
were mostly split for review purposes, hence commit them all at once.
They can still be backed up using the patch files, if necessary.
