Currently, the MMCONF Kconfigs only support the Enhanced Configuration
Access mechanism (ECAM) method for accessing the PCI config address
space. Some platforms have a different way of mapping the PCI config
space to memory. This patch renames the following configs to
make it clear that these configs are ECAM-specific:
- NO_MMCONF_SUPPORT --> NO_ECAM_MMCONF_SUPPORT
- MMCONF_SUPPORT --> ECAM_MMCONF_SUPPORT
- MMCONF_BASE_ADDRESS --> ECAM_MMCONF_BASE_ADDRESS
- MMCONF_BUS_NUMBER --> ECAM_MMCONF_BUS_NUMBER
- MMCONF_LENGTH --> ECAM_MMCONF_LENGTH

Create `FIXED_RCBA_MMIO_BASE` and use it everywhere, except in cases
where a pointer cast would be necessary. Instances in Sandy Bridge MRC
code were left as-is intentionally, so as not to collide with another
cleanup patch train.
Tested with BUILD_TIMELESS=1, these boards remain identical:
- Asus P8Z77-V LX2
- Packard Bell MS2290

Move all Q35 register definitions into the q35.h header. Note that real
hardware does not have EXT_TSEG_MBYTES, because it is QEMU-specific.

The Kconfig lint tool checks for cases of the code using BOOL type
Kconfig options directly instead of with CONFIG() and will print out
warnings about it. It gets confused by these references in comments
and strings. To fix it so that it can find the real issues, just
update these as we would with real issues.

The QEMU machine "PC" doesn't support MCFG.
Die after console init if the user selected the wrong qemu machine
and print a message to use the correct machine type.
Without this patch ramstage dies with non-helpful message:
"get_pbus: dev is NULL!"

Useful for testing stuff in C_ENVIRONMENT_BOOTBLOCK, like
VBOOT with separate verstage.
Changes:
* Use symbols to set up CAR and STACK
* Zero CAR area
* Move BIST failure checking to cpu folder
* Rename functions where necessary
Tested:
* qemu-2.11.2 machine pc
* qemu-2.11.2 machine q35
Test result:
* BIST error reporting is still working.
* Console starts in bootblock
* SeaBios 1.11.2 as payload is still working

This change adds and updates headers in all of the mainboard files that
had missing or unrecognized headers. After this goes in, we can turn on
lint checking for headers in all mainboard directories.

Doing PCI config operations via MMIO window by default is a
requirement, if supported by the platform. This means chipset
or CPU code must enable MMCONF operations early in bootblock
already, or before platform-specific romstage entry.
Platforms are allowed to have NO_MMCONF_SUPPORT only in the
case it is actually not implemented in the silicon.

Change all PCI configuration accesses to MMIO in qemu-q35
emulation
To enable MMIO style access, add (move) explicit PCI IO config write
in the bootblock. As there is no northbridge/x/x/bootblock.c
file, a mainboard/x/x/bootblock.c file is added for this purpose.
