Some PCI capabilities should only be enabled if it is available not
only on a device, but also all bridge upstream of it. Checking only
the device and the bridge just above it may not be enough.

This adds SPDX-License-Identifiers to all of the files in src/include
that are missing them or have unrecognized identifiers.
Files that were written specifically for coreboot and don't have license
information are licensed GPL-2.0-only, which is the license for the
overall coreboot project.
Files that were sourced from Linux are similarly GPL-2.0-only.
The cpu/power files were committed with source that was licensed as
GPL-2.0-or-later, so presumably that's the license for that entire
commit.
The final file, vbe.h gives a pointer to the BSD-2-Clause license
at opensource.org.

I was bugged by spurious "Failed to enable LTR" messages for years.
Looking at the the current algorithm, it is flawed in multiple ways:
* It looks like the author didn't know they implemented a
recursive algorithm (pciexp_enable_ltr()) inside another
recursive algorithm (pciexp_scan_bridge()). Thus, at every
tree level, everything is run again for the whole sub-
tree.
* LTR is enabled no matter if `.set_ltr_max_latencies` is
implemented or not. Leaving the endpoints' LTR settings
at 0: They are told to always report zero tolerance.
In theory, depending on the root-complex implementation,
this may result in higher power consumption than without
LTR messages.
* `.set_ltr_max_latencies` is only considered for the direct
parent of a device. Thus, even with it implemented, an
endpoint below a (non-root) bridge may suffer from the 0
settings as described above.
* Due to the double-recursive nature, LTR is enabled starting
with the endpoints, then moving up the tree, while the PCIe
spec tells us to do it in the exact opposite order.
With the current implementation of pciexp_scan_bridge(), it is
hard to hook anything in that runs for each device from top to
bottom. So the proposed solution still adds some redundancy:
First, for every device that uses pciexp_scan_bus(), we enable
LTR if possible (see below). Then, when returning from the bus-
scanning recursion, we enable LTR for every device and configure
the maximum latencies (if supported). The latter runs again on
all bridges, because it's hard to know if pciexp_scan_bus() was
used for them.
When to enable LTR:
* For all devices that implement `.set_ltr_max_latencies`.
* For all devices below a bridge that has it enabled already.

As an intermediate step for CB:45150, add an additional Kconfig option
which is used to configure bus mastering for any devices and use
PCI_ALLOW_BUS_MASTER to allow coreboot setting the bus mastering bit in
general.

This change adds a helper function `pci_dev_is_wake_source()` that
checks PME_STATUS and PME_ENABLE bits in PM control and status
register to determine if the given device is the source of wake.

The bus master bit is set at many places in coreboot's code, but the
reason for that is not quite clear. We examined not setting the
bus master bit whereever possible and tried booting without it,
which worked fine for internal PCI devices but not for PCIe. As a PCIe
device we used a Samsung M.2 NVMe SSD.
For security reasons, we would like to disable bus mastering where
possible. Depending on the device, bus mastering might get enabled
by the operating system (e.g. for iGPU) and it might be required for
some devices to work properly. However, the idea is to leave it disabled
and configure the IOMMU first before enabling it.
To have some sort of "backwards compatibility", add a method which
configures bus mastering based on an additional config option. Since
CB:42460 makes usage of this treewide, enable it by default to keep the
current behaviour for now.
Tested with Siemens/Chili, a Coffee Lake based platform.

A function pci_dev_disable_bus_master() is created. This function
can be used to disable Thunderbolt PCIe root ports, bridges and
devices for Vt-d based security platform at end of boot service.

We have some use for PCI bridge configurations and
resets in romstages, so expose them.

In the current state of the tree we do not utilise the

ops.
This change effectively inlines all PCI config accessors

These are defined for __SIMPLE_DEVICE__ when PCI
enumeration has not happened yet. These should not
really try to probe devices other than those on bus 0.
It's hard to track but there maybe cases of southbridge
being located on bus 2 and available for configuration, so
I rather leave the code unchanged. Just move these out of
arch/io.h because they cause build failures if one attempts
to include <arch/pci_ops.h> before <arch/io.h>.
There are two direct copies for ROMCC bootblocks to
avoid inlining them elsewhere.

The feature is used to enable PCI MMIO accesses behind
PCIe links (or bridges) before PCI enumeration has been
completed.
Add the feature for bootblock, verstage and postcar, it
is required with add-on PCIe serial cards for early
console output. It's up to the board specific code to
configure PCIe root port prior to calling console_init()
for this to work.
Remove feature from ramstage, it bypasses any resource
allocations and bus number assignments.
For the moment PCI configuration support before ramstage
is available only on ARCH_X86.
Also switch from device_t to pci_devfn_t.

This patch uses struct device explicitly for the ramstage functions
as that's the actual type it's working on. Additionally, the
declarations for types and functions are fully exposed so that
compliation units don't have to guard certain functions from use
because it's being compiled for multiple stages.

Unfortunately coreboot has to deal with ROMCC's short comings which has
lead to a little bit of confusion due to typedefs. Essentially, coreboot
defines four typedefs:
* 'typedef struct device * device_t' in ramstage not in SIMPLE_DEVICE mode
* 'typedef u32 device_t' in romstage or when SIMPLE_DEVICE is defined
* 'typedef u32 pnp_devfn_t'
* 'typedef u32 pci_devfn_t'
Some early functions make use of 'device_t' over 'pci_devfn_t' and since
the C type-checker does not enforce typedefs to the same type 'u32'
these are never noticed. Fix these so that 'device_t' does not conflict
in romstage for later work. We later plan to have 'pnp_devfn_t' and
'pci_devfn_t' as the only variants of 'u32' and 'device_t' to be a
struct pointer type exclusively.

The PCI Specification states that devices that implement
a bridge and a secondary bus must swizzle (rotate) the
interrupt pins according to the table below:
Child Dev # Child PIN Parent PIN
0,4,8,12... A/B/C/D A/B/C/D
1,5,9,13... A/B/C/D B/C/D/A
2,6,10,14.. A/B/C/D C/D/A/B
3,7,11,15.. A/B/C/D D/A/B/C
Which is also described by this equation:
PIN_parent = (Pin_child + Dev_child) % 4
When a device is found and its bus number is greater than 0,
it is on a bridge and needs to be swizzled. Following the
string of parents up to the root bus and swizzling as we go
gives us the desired swizzling result. When BIOS_SPEW is
defined, it will print out each step of the swizzling process.

None of the PCI bridge management here is specific to the PCI UART
device/function. Also the Kconfig variable defaults are not globally
valid, fill samsung/lumpy with working values.

The tests for __PRE_RAM__ or __SMM__ were repeatedly used
for detection if dev->ops in the devicetree are not available
and simple device model functions need be used.
If a source file build for ramstage had __PRE_RAM__ inserted
at the beginning, the struct device would no longer match the
allocation the object had taken. This problem is fixed by
replacing such cases with explicit __SIMPLE_DEVICE__.

Taking device_t as a parameter, this allows to alter the PCI config
access handlers. This is useful to add tracing of PCI config writes
for devices having problems to initialise correctly.
On older AMD platform PCI MMIO may not be able to fully configure all
PCI devices/nodes, while MMIO_SUPPORT_DEFAULT would be preferred due
to its atomic nature. So those can be forced to IO config instead.

Here's the great news: From now on you don't have to worry about
hitting the right io.h include anymore. Just forget about romcc_io.h
and use io.h instead. This cleanup has a number of advantages, like
you don't have to guard device/ includes for SMM and pre RAM
anymore. This allows to get rid of a number of ifdefs and will
generally make the code more readable and understandable.
Potentially in the future some of the code in the io.h __PRE_RAM__
path should move to device.h or other device/ includes instead,
but that's another incremental change.

We used to allow mainboards to override subsystems using
mainboard_pci_subsystem_vendor_id and mainboard_pci_subsystem_device_id.
Mechanisms have changed and the only occurrence of these names is in
the header.

We thought about two ways to do this change. The way we decided to try
was to
1. drop all ops from devices in romstage
2. constify all devices in romstage (make them read-only) so we can
compile static.c into romstage
3. the device tree "devices" can be used to read configuration from
the device tree (and nothing else, really)
4. the device tree devices are accessed through struct device * in
romstage only. device_t stays the typedef to int in romstage
5. Use the same static.c file in ramstage and romstage
We declare structs as follows:

ROMSTAGE_CONST is const in romstage and empty in ramstage; This
forces all of the device tree into the text area.

PCI Type 2 config was a strange and never-used config mechanism.
It is unlikely that in the 13 years of coreboot's existence that
type 2 was ever used; it just made life complicated for everyone.
It lived long enough in coreboot to be replaced by mmioconf.
Prior to making the device tree visible in romstage we want to
get rid of type2.
Delete two files we don't need any more (yay!).
Replace two functions with one: pci_config_default, which returns
a pointer to the default config method. At some future time this
may change to mmio but for now it is old type1 style.

Many PCI devices share the very same driver despite having different
PCI device IDs, which causes a lot of copy and paste of driver
definitions.
This change introduces a way to specify the array of acceptable
device IDs in a single driver entry. As an example the Intel
{Sandy|Ivy} Bridge SATA driver is being modified to use a single
driver structure for all different SATA controller flavors, a few
more Ivy Bridge IDs are being added as well.

TEST=manual
. modified coreboot brought up an Ivy Bridge platform all the
way to Linux login screen.

during early coreboot_ram), pci_{read,write}_config{8,16,32} will die().
This patch changes pci_{read,write}_config{8,16,32} to use the existing
PCI access method autodetection infrastructure instead of die()ing.
Until r4340, any usage of pci_{read,write}_config{8,16,32} in
coreboot_ram before the device tree was set up resulted in either a
silent hang or a NULL pointer dereference. I changed the code in r4340
to die() properly with a loud error message. That still was not perfect,
but at least it allowed people to see why their new ports died.
Still, die() is not something developers like to see, and thus a patch
to automatically pick a sensible default instead of dying was created.
Of course, handling PCI access method selection automatically for
fallback purposes has certain limitations before the device tree is set
up. We only check if conf1 works and use conf2 as fallback. No further
tests are done.
This patch enables cleanups and readability improvements in early
coreboot_ram code:
Without this patch:

For some busses like i2c it is non-sense and we don't want it.
- Set the resource limits for pnp devices resources.
- Improve the resource size detection for pnp devices.
- Added a configuration register to amd8111_ide.c so we can enable/disable individual ide channels
- Added a header file to hold the prototype of isa_dma_init
- Fixed most of the superio chips so the should work now, the via superio pci device is the exception.
- The code compiles and runs so it is time for me to go to bed.
