There are four requirements for the SMI to hit a printk()
this commit now removes.
Build must have DEBUG_SMI=y, otherwise any printk() is a no-op
inside SMM.
ASL must have a TRAP() with argument 0x99 or 0x32 for SMIF value.
Platform needs to have IO Trap #3 enabled at IO 0x800.
The SMI monitor must call io_trap_handler for IO Trap #3.
At the moment, only getac/p470 would meet the above criteria
with TRAP(0x32) in its DSDT _INI method. The ASL ignores any
return value of TRAP() calls made.
A mainboard IO trap handler should have precedence over
a southbridge IO trap handler. At the moment we seem to have
no cases of the latter to support, so remove the latter.

This change provides hooks for the SoC so it can perform any
initialization and cleanup in the SMM handler.
For example, if we have a UART enabled firmware with DEBUG_SMI, the UART
controller could have been powered off by the OS. In this case we need
to power on the UART when entering SMM, and then power it off before we
exit. If the OS had the UART enabled when entering SMM, we should
snapshot the UART register state, and restore it on exit. Otherwise we
risk clearing some interrupt enable bits.

This change will allow the SMI handler to write to the cbmem console
buffer. Normally SMIs can only be debugged using some kind of serial
port (UART). By storing the SMI logs into cbmem we can debug SMIs using
`cbmem -1`. Now that these logs are available to the OS we could also
verify there were no errors in the SMI handler.
Since SMM can write to all of DRAM, we can't trust any pointers
provided by cbmem after the OS has booted. For this reason we store the
cbmem console pointer as part of the SMM runtime parameters. The cbmem
console is implemented as a circular buffer so it will never write
outside of this area.

TEST=Boot non-serial FW with DEBUG_SMI and verified SMI messages are
visible when running `cbmem -1`. Perform a suspend/resume cycle and
verify new SMI events are written to the cbmem console log.

This commit adds a method called `mainboard_smi_finalize` which provides
a mechanism for a mainboard to execute some code as part of the finalize
method in the SMM stage before SoC does its finalization.

Use the same stack location during relocation as for the permanent
handler.
When the number of CPUs is too large the stacks during relocation
don't fit inside the default SMRAM segment at 0x30000. Currently the
code would just let the CPU stack base grow downwards outside of the
default SMM segment which would corrupt lower memory if S3 is
implemented.
Also update the comment on smm_module_setup_stub().

Implement x2apic mode as existing code only supports apic mode.
Use info from LAPIC_BASE_MSR (LAPIC_BASE_MSR_X2APIC_MODE) to check
if apic mode or x2apic mode and implement x2apic mode according to
x2apic specfication.

The permanent handler module argument 'save_state_size' now holds the
meaning of the real save state size which is then substracted from the
CPUs save state 'top' to get the save state base.
TESTED with qemu Q35 on x86_64 where the stub size exceeds the AMD64
save state size.

With the smm_module_loaderv2 the save state map is not linear so copy
a map from ramstage into the smihandler.
TESTED on QEMU q35: Both SMMLOADER V1 and V2 handle save states properly.

The parameters that the permanent handler requires are pushed directly
to the permanent handlers relocatable module params.
The paremeters that the relocation handler requires are not passed on
via arguments but are copied inside the ramstage. This is ok as the
relocation handler calls into ramstage.

struct smm_loader_params is a struct that is passed around in the
ramstage code to set up either the relocation handler or the permanent
handler. At the moment no parameters in the stub 'smm_runtime' are
referenced so it can be dropped. The purpose is to drop the
smm_runtime struct from the stub as it is already located in the
permanent handler.

These stub params need to be synced with the code in smm_stub.S and
are consumed by both the smmloader and smmloader_v2. So it is better
to have the definition located in one place.

NOTE: This commit does NOT introduce a new security model. It uses the
same page tables as the remaining firmware does.
This can be a security risk if someone is able to manipulate the
page tables stored in ROM at runtime. USE FOR TESTING ONLY!
Tested on Lenovo T410 with additional x86_64 patches.

Xeon-SP Skylake Scalable Processor can have 36 CPU threads (18 cores).
Current coreboot SMM is unable to handle more than ~32 CPU threads.
This patch introduces a version 2 of the SMM module loader which
addresses this problem. Having two versions of the SMM module loader
prevents any issues to current projects. Future Xeon-SP products will
be using this version of the SMM loader. Subsequent patches will
enable board specific functionality for Xeon-SP.
The reason for moving to version 2 is the state save area begins to
encroach upon the SMI handling code when more than 32 CPU threads are
in the system. This can cause system hangs, reboots, etc. The second
change is related to staggered entry points with simple near jumps. In
the current loader, near jumps will not work because the CPU is jumping
within the same code segment. In version 2, "far" address jumps are
necessary therefore protected mode must be enabled first. The SMM
layout and how the CPUs are staggered are documented in the code.
By making the modifications above, this allows the smm module loader to
expand easily as more CPU threads are added.
TEST=build for Tiogapass platform under OCP mainboard. Enable the
following in Kconfig.
select CPU_INTEL_COMMON_SMM
select SOC_INTEL_COMMON_BLOCK_SMM
select SMM_TSEG
select HAVE_SMI_HANDLER
select ACPI_INTEL_HARDWARE_SLEEP_VALUES
Debug console will show all 36 cores relocated. Further tested by
generating SMI's to port 0xb2 using XDP/ITP HW debugger and ensured all
cores entering and exiting SMM properly. In addition, booted to Linux
5.4 kernel and observed no issues during mp init.

The call made at mp_ops.post_mp_init() generally
uses four different names. Unify these with followups.
smm_southbridge_enable(SMI_EVENTS)
smm_southbridge_enable_smi()
hudson_enable_smi_generation()
enable_smi_generation()
Furthermore, some platforms do not enable power button
SMI early. It may be preferred to delay the enablement,
but fow now provide global_smi_enable_no_pwrbtn() too.

They're listed in AUTHORS and often incorrect anyway, for example:
- What's a "Copyright $year-present"?
- Which incarnation of Google (Inc, LLC, ...) is the current
copyright holder?
- People sometimes have their editor auto-add themselves to files even
though they only deleted stuff
- Or they let the editor automatically update the copyright year,
because why not?
- Who is the copyright holder "The coreboot project Authors"?
- Or "Generated Code"?
Sidestep all these issues by simply not putting these notices in
individual files, let's list all copyright holders in AUTHORS instead
and use the git history to deal with the rest.

To mitigate against sinkhole in software which is required on
pre-sandybridge hardware, the smm entry point needs to check if the
LAPIC base is between smbase and smbase + smmsize. The size needs to
be available early so add them to the relocatable module parameters.
When the smmstub is used to relocate SMM the default SMM size 0x10000
is provided. On the permanent handler the size provided by
get_smm_info() is used.

This update is a combination of all four of the patches so that the
commit can be done without breaking parts of coreboot. This possible
breakage is because of the cross-dependencies between the original
separate patches would cause failure because of data structure changes.
security/intel/stm
This directory contains the functions that check and move the STM to the
MSEG, create its page tables, and create the BIOS resource list.
The STM page tables is a six page region located in the MSEG and are
pointed to by the CR3 Offset field in the MSEG header. The initial
page tables will identity map all memory between 0-4G. The STM starts
in IA32e mode, which requires page tables to exist at startup.
The BIOS resource list defines the resources that the SMI Handler is
allowed to access. This includes the SMM memory area where the SMI
handler resides and other resources such as I/O devices. The STM uses
the BIOS resource list to restrict the SMI handler's accesses.
The BIOS resource list is currently located in the same area as the
SMI handler. This location is shown in the comment section before
smm_load_module in smm_module_loader.c
Note: The files within security/intel/stm come directly from their
Tianocore counterparts. Unnecessary code has been removed and the
remaining code has been converted to meet coreboot coding requirements.

Its spreading copies got out of sync. And as it is not a standard header
but used in commonlib code, it belongs into commonlib. While we are at
it, always include it via GCC's `-include` switch.
Some Windows and BSD quirk handling went into the util copies. We always
guard from redefinitions now to prevent further issues.

If CPU 0's stack grows to large, it will overflow into CPU 1's stack.
If CPU 0 is handling the interrupt then CPU 1 should be in an idle loop.
When the stack overflow occurs it will override the return pointer for
CPU 1, so when CPU 0 unlocks the SMI lock, CPU 1 will attempt to return
to a random address.
This method is not foolproof. If code allocates some stack variables
that overlap with the canary, and if the variables are never set, then
the canary will not be overwritten, but it will have been skipped. We
could mitigate this by adding a larger canary value if we wanted.
I chose to use the stack bottom pointer value as the canary value
because:
* It will change per CPU stack.
* Doesn't require hard coding a value that must be shared between the
.S and .c.
* Passing the expected canary value as a parameter felt like overkill.
We can explore adding other methods of signaling that a stack overflow
had occurred in a follow up. I limited die() to debug only because
otherwise it would be very hard to track down.
TEST=built on grunt with a small and large stack size. Then verified
that one causes a stack overflow and the other does not.

The SMM module loader code was guarded by CONFIG_SMM_TSEG,
however that's not necessary. It's up to the chipset to take
advantage of the SMM module loading. It'll get optimized out
if the code isn't used anyway so just expose the declarations.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

Most Baytrail based devices MMIO registers are reported in ACPI
space and the device's PCI config space is disabled. The PCI config
space is required for many "legacy" OSs that don't have the ACPI
driver loading mechanism. Depthcharge signals the legacy boot
path via the SMI 0xCC and the coreboot SMI handler can switch the
device specific registers to re-enable PCI config space.

Start using the rmodtool for generating rmodules.
rmodule_link() has been changed to create 2 rules:
one for the passed in <name>, the other for creating
<name>.rmod which is an ELF file in the format of
an rmodule.
Since the header is not compiled and linked together
with an rmodule there needs to be a way of marking
which symbol is the entry point. __rmodule_entry is
the symbol used for knowing the entry point. There
was a little churn in SMM modules to ensure an
rmodule entry point symbol takes a single argument.

Certain CPUs require the default SMM region to be backed up
on resume after a suspend. The reason is that in order to
relocate the SMM region the default SMM region has to be used.
As coreboot is unaware of how that memory is used it needs to
be backed up. Therefore provide a common method for doing this.

This commit adds a new Kconfig option for the LynxPoint
southbridge that will have coreboot route all of the USB
ports to the XHCI controller in the finalize step (i.e.
after the bootloader) and disable the EHCI controller(s).
Additionally when doing this the XHCI USB3 ports need
to be put into an expected state on resume in order to make
the kernel state machine happy.
Part of this could also be done in depthcharge but there
are also some resume-time steps required so it makes sense
to keep it all together in coreboot.
This can theoretically save ~100mW at runtime.
Verify that the EHCI controller is not found in Linux and
that booting from USB still works.

With the LynxPoint chipset there are more than 16
possible GPIOs that can trigger an SMI so we need
a mainboard handler that can support this.
There are only a handful of users of this function
so just change them all to use the new prototype.

Some of the functions called from assembly assume the standard
x86 32-bit ABI of passing all arguments on the stack. However,
that calling ABI can be changed by compiler flags. In order to
protect against the current implicit calling convention annotate
the functions called from assembly with the cdecl function
attribute. That tells the compiler to use the stack based parameter
calling convention.

Currently the OS is free to use the memory located at the default
SMRAM space because it is not marked reserved in the e820. This can
lead to memory corruption on S3 resume because SMM setup doesn't save
this range before using it to relocate SMRAM.

Add support for SMM modules by leveraging the RMODULE lib. This allows
for easier dynamic SMM handler placement. The SMM module support
consists of a common stub which puts the executing CPU into protected
mode and calls into a pre-defined handler. This stub can then be used
for SMM relocation as well as the real SMM handler. For the relocation
one can call back into coreboot ramstage code to perform relocation in
C code.
The handler is essentially a copy of smihandler.c, but it drops the TSEG
differences. It also doesn't rely on the SMM revision as the cpu code
should know what processor it is supported.
Ideally the CONFIG_SMM_TSEG option could be removed once the existing
users of that option transitioned away from tseg_relocate() and
smi_get_tseg_base().
The generic SMI callbacks are now not marked as weak in the
declaration so that there aren't unlinked references. The handler
has default implementations of the generic SMI callbacks which are
marked as weak. If an external compilation module has a strong symbol
the linker will use that instead of the link one.
Additionally, the parameters to the generic callbacks are dropped as
they don't seem to be used directly. The SMM runtime can provide the
necessary support if needed.

According to both Haswell and the SandyBridge/Ivybridge
BWGs the save state area actually starts at 0x7c00 offset
from 0x8000. Update the em64t101_smm_state_save_area_t
structure and introduce a define for the offset.
Note: I have no idea what eptp is. It's just listed in the
haswell BWG. The offsets should not be changed.

Using global variables with the TSEG is a bad idea because
they are not relocated properly right now. Instead make
the variables static and add accessor functions for the
rest of SMM to use.
At the same time drop the tcg/smi1 pointers as they are
not setup or ever used. (the debug output is added back
in a subsequent commit)

There are enough differences that it is worth defining the
proper map for the sandybridge/ivybridge CPUs. The state
save map was not being addressed properly for TSEG and
needs to use the right offset instead of pointing in ASEG.
To do this properly add a required southbridge export to
return the TSEG base and use that where appropriate.

rename from mainboard_apm_cnt to mainboard_smi_apmc to match the function
naming scheme of the other handlers. Add prototype for mainboard_smi_sleep
(mainboard specific S3 sleep handlers in SMM) that is required by Sandybridge.

Traditionally coreboot's SMM handler runs in ASEG (0xa0000),
"behind" the graphics memory. This approach has two issues:
- It limits the possible size of the SMM handler (and the
number of CPUs supported in a system)
- It's not considered a supported path anymore in newer CPUs.

Move the SMM MSR init to a code run per CPU. Introduce global SMM_BASE define,
later all 0xa0000 could be changed to use it. Remove the unnecessary test if
the smm_init routine is called once (it is called by BSP only) and also remove
if lock bit is set becuase this bit is cleared by INIT it seems.
Add the defines for fam10h and famfh to respective files, we do not have any
shared AMD MSR header file.
Tested on M2V-MX SE with dualcore CPU.
