The `location` member of `struct boot_state_callback` is conditionally
guarded depending on `CONFIG(DEBUG_BOOT_STATE)` using preprocessor. It
is probably intended to save some space when the `location` strings do
not get printed. However, directly using the `location` member without
any guards will cause a compile-time error. Plus, preprocessor-guarded
code gets nasty really quickly.
In order to minimise preprocessor usage, introduce the `bscb_location`
inline helper function, which transforms the compile-time error into a
link-time error. It is then possible to substitute preprocessor guards
with an ordinary C `if` statement.

They're listed in AUTHORS and often incorrect anyway, for example:
- What's a "Copyright $year-present"?
- Which incarnation of Google (Inc, LLC, ...) is the current
copyright holder?
- People sometimes have their editor auto-add themselves to files even
though they only deleted stuff
- Or they let the editor automatically update the copyright year,
because why not?
- Who is the copyright holder "The coreboot project Authors"?
- Or "Generated Code"?
Sidestep all these issues by simply not putting these notices in
individual files, let's list all copyright holders in AUTHORS instead
and use the git history to deal with the rest.

The bootstate machine allows one to schedule work at the
boundaries of each state. However, there are no priorities by
design. As such if there are things that need to be performed
that are interdependent between callbacks there's no way to
do that aside from explicitly putting the call in one of the
callbacks.
This situation arises around BS_OS_RESUME, BS_PAYLOAD_LOAD,
and BS_PAYLOAD_BOOT as those are the states where coreboot is
about to exit. As such, provide an architecture specific hook
at these key places so that one is guaranteed any work done
in arch_bootstate_coreboot_exit() is after all callbacks in
the state machine.

Add the DEBUG_BOOT_STATE Kconfig value to enable boot state debugging.
Update include/bootstate.h and lib/hardwaremain.c to honor this value.
Add a dashed line which displays between the states.
Testing on Galileo:

It is silly to have a single header to declare the main()
symbol, however some of the arches provided it while
lib/bootblock.c relied on the arch headers to declare it. Just
move the declaration into its own header file and utilize it.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

The BOOT_STATE_INIT_ENTRY macro can only be used in ramstage, however
the current state of the header meant bad build errors in non-ramstage.
Therefore, people had to #ifdef in the source. Remove that requirement.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

The GCC 4.9.2 update showed that the boot_state_init_entry
structures were being padded and assumed to be aligned in to an
increased size. The bootstate scheduler for static entries,
boot_state_schedule_static_entries(), was then calculating the
wrong values within the array. To fix this just use a pointer to
the boot_state_init_entry structure that needs to be scheduled.
In addition to the previous issue noted above, the .bs_init
section was sitting in the read only portion of the image while
the fields within it need to be writable. Also, the
boot_state_schedule_static_entries() was using symbol comparison
to terminate a loop which in C can lead the compiler to always
evaluate the loop at least once since the language spec indicates
no 2 symbols can be the same value.

In order to properly sequence the boot state machine it's
important that outside code can block the transition from
one state to the next. When timers are not involved there's
no reason for any of the existing code to block a state
transition. However, if there is a timer callback that needs to
complete by a certain point in the boot sequence it is necessary
to place a block for the given state.
To that end, 4 new functions are added to provide the API for
blocking a state.
1. boot_state_block(boot_state_t state, boot_state_sequence_t seq);
2. boot_state_unblock(boot_state_t state, boot_state_sequence_t seq);
3. boot_state_current_block(void);
4. boot_state_current_unblock(void);

It's helpful to provide a distinct state that affirmatively
describes that OS resume will occur. The previous code included
the check and the actual resuming in one function. Because of this
grouping one had to annotate the innards of the ACPI resume
path to perform specific actions before OS resume. By providing
a distinct state in the boot state machine the necessary actions
can be scheduled accordingly without modifying the ACPI code.

Many of the boot state callbacks can be scheduled at compile time.
Therefore, provide a way for a compilation unit to inform the
boot state machine when its callbacks should be called. Each C
module can export the callbacks and their scheduling requirements
without changing the shared boot flow code.

The boot flow currently has a fixed ordering. The ordering
is dictated by the device tree and on x86 the PCI device ordering
for when actions are performed. Many of the new machines and
configurations have dependencies that do not follow the device
ordering.
In order to be more flexible the concept of a boot state machine
is introduced. At the boundaries (entry and exit) of each state there
is opportunity to run callbacks. This ability allows one to schedule
actions to be performed without adding board-specific code to
the shared boot flow.
