These two identifiers were always very confusing. We're not filling and
injecting generators. We are filling SSDTs and injecting into the DSDT.
So drop the `_generator` suffix. Hopefully, this also makes ACPI look a
little less scary.

The chip.h for this driver was updated to add a reset GPIO, but did
not add the required #include of <arch/acpi_device.h>. This likely
still compiled because other chip.h files included before it may have
included it themselves.

As discussed on the mailing list and voted upon, the coreboot project
is going to move the majority of copyrights out of the headers and into
an AUTHORS file. This will happen a bit at a time, as we'll be unifying
license headers at the same time.
Updated Authors file is in a separate commit.

scan_usb_bus() and root_dev_scan_bus() had the very same implementation.
So rename the latter to scan_static_bus() and use that for both cases.

add it when it is missing.
Also extra lines removed, or added just before local includes.

The Linux kernel can use the ACPI _PLD group information to
determine peer ports. Currently to define the group information
the devicetree must provide a complete _PLD structure. This
change pulls the group information into a separate structure that
can be defined in devicetree. This makes it easier to set for
USB devices in devicetree that do not need a full custom PLD.
This was tested on a sarien board with the USB devices defined
by verifying that the USB 2/3 ports are correctly identified
with their peer in sysfs.

Add a support for generating USB port descriptors for ACPI based
on their definition by the board in devicetree.cb. This will
generate a _UPC and _PLD for each port, using a generic _PLD by
default. The _PLD can also be customized for more accurate
descriptions if necessary.
This sample devictree.cb shows a USB 2.0 type-A port behind a root
hub connected to an xHCI controller:
