Some of the chip.h files in the tree are missing the include guards.
This patch adds them in order to avoid potential redefinions of symbols
contained in these headers, when they are included multiple times in
static.c generated by sconfig.

The exceptions are for:

- nvramtool (more complicated header)

As discussed on the mailing list and voted upon, the coreboot project
is going to move the majority of copyrights out of the headers and into
an AUTHORS file. This will happen a bit at a time, as we'll be unifying
license headers at the same time.
Updated Authors file is in a separate commit.

This change list adds imon and vmon slot numbers as params for
Maxim 98927 driver. These values are looked up in the kernel driver
to confiure IV feedback for audio playback on speakers.
BUG=b:36724448
TEST=After boot, the register dump for Max98927 codecs should have
imon and vmon slots numbers set in 0x1e register.

Maxim 98927 kernel driver requires entries in the ACPI SSDT table,
add a SSDT generator as part of this driver.
