As discussed on the mailing list and voted upon, the coreboot project
is going to move the majority of copyrights out of the headers and into
an AUTHORS file. This will happen a bit at a time, as we'll be unifying
license headers at the same time.
Updated Authors file is in a separate commit.

On x86, change the type of the address parameter in
read8()/read16/read32()/write8()/write16()/write32() to be a
pointer, instead of unsigned long.

Some LPC initialiation can save some lines of code when being able
to use the functions `io_apic_read()` and `io_apic_write()`.
As these two functions are now public, remove them from the generic
driver as otherwise we get a build errors like the following.
