Currently, the MMCONF Kconfigs only support the Enhanced Configuration
Access mechanism (ECAM) method for accessing the PCI config address
space. Some platforms have a different way of mapping the PCI config
space to memory. This patch renames the following configs to
make it clear that these configs are ECAM-specific:
- NO_MMCONF_SUPPORT --> NO_ECAM_MMCONF_SUPPORT
- MMCONF_SUPPORT --> ECAM_MMCONF_SUPPORT
- MMCONF_BASE_ADDRESS --> ECAM_MMCONF_BASE_ADDRESS
- MMCONF_BUS_NUMBER --> ECAM_MMCONF_BUS_NUMBER
- MMCONF_LENGTH --> ECAM_MMCONF_LENGTH

With the recent switch to SMM module loader v2, the size of the SMM for
module google/volteer increased to above 64K in size, and thus failed to
install the permanent SMM handler. Turns out, the devicetree is all
pulled into the SMM build because of elog, which calls
`pci_dev_is_wake_source`, and is the only user of `struct device` in
SMM. Changing this function to take a pci_devfn_t instead allows the
linker to remove almost the entire devicetree from SMM (only usage left
is when disabling HECI via SMM).

TEST=Verify loaded program size of `smm.elf` for google/volteer is
almost ~50% smaller.

Caller needs to take into account that bus numbers may
have not been assigned yet. Same issue existed before
with early ramstage and mostly does not cause problems
when used with static devices on bus 0.

The effect of pointer aliasing on writes is that any data on CPU
registers that has been resolved from (non-const and non-volatile)
memory objects has to be discarded and resolved. In other words, the
compiler assumes that a pointer that does not have an absolute value
at build-time, and is of type 'void *' or 'char *', may write over
any memory object.
Using a unique datatype for MMIO writes makes the pointer to _not_
qualify for pointer aliasing with any other objects in memory. This
avoid constantly resolving the PCI MMCONF address, which is a derived
value from a 'struct device *'.

Drop the bus parameter, we do not use it.
It would still be possible to do per-bus selection
by evaluating the bus number, but currently we do
not have need for that either.

Fixes compilation on platforms that do neither have MMCONF_SUPPORT
nor NO_MMCONF_SUPPORT (for example: all non-x86) but select PCI.

Doing PCI config operations via MMIO window by default is a
requirement, if supported by the platform. This means chipset
or CPU code must enable MMCONF operations early in bootblock
already, or before platform-specific romstage entry.
Platforms are allowed to have NO_MMCONF_SUPPORT only in the
case it is actually not implemented in the silicon.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

Taking device_t as a parameter, this allows to alter the PCI config
access handlers. This is useful to add tracing of PCI config writes
for devices having problems to initialise correctly.
On older AMD platform PCI MMIO may not be able to fully configure all
PCI devices/nodes, while MMIO_SUPPORT_DEFAULT would be preferred due
to its atomic nature. So those can be forced to IO config instead.
