This patch renames X86_AMD_INIT_SIPI Kconfig to leverage
the same logic (to skip 2nd SIPI and reduce delay between
INIT and SIPI while perform AP initialization) even on
newer Intel platform.

Each CPU/SoC checks the return value of the mp_init_with_smm and prints
the same error message if it wasn't successful, so move this check and
printk to mp_init_with_smm. For this the original mp_init_with_smm
function gets renamed to do_mp_init_with_smm and a new mp_init_with_smm
function is created which then calls do_mp_init_with_smm, prints the
error if it didn't return CB_SUCCESS and passes the return value of
do_mp_init_with_smm to its caller.
Since no CPU/SoC code handles a mp_init_with_smm failure apart from
printing a message, also add a comment at the mp_init_with_smm call
sites that the code might want to handle a failure.

Using cb_err as return type of mp_run_on_aps, mp_run_on_all_aps,
mp_run_on_all_cpus and mp_park_aps clarifies the meaning of the
different return values. This patch also adds the types.h include that
provides the definition of the cb_err enum and checks the return value
of all 4 functions listed above against the enum values instead of
either checking if it's non-zero or less than zero to handle the error
case.

We now pre-populate cpu_info before jumping to the C handler. We no
longer need this parameter.
I moved the stack alignment closer to the actual invocation of the C
handler so it's easier to reason about.

When CONFIG_X86_AMD_INIT_SIPI was set, the second/final SIPI that
afterwards checks if all APs have checked in was skipped and if it got
so far, start_aps returned CB_SUCCESS despite not having checked if all
APs had checked in after the SIPI. This patch makes start_aps skip the
first SIPI in the CONFIG_X86_AMD_INIT_SIPI case so we use the proper
timeouts and error handling for the final and this case only SIPI and
signal the caller an error when not all APs have checked in after the
SIPI.
A timeless build for lenovo/x230 which is a mainboard that doesn't
select X86_AMD_INIT_SIPI results in identical binary, so this doesn't
change the behavior of the !X86_AMD_INIT_SIPI case.

Since during AP startup it's not guaranteed that no AP console output
will be printed between consecutive printk calls in send_sipi_to_aps,
add a new line character to all printks to make sure to have the outputs
from the APs on separate lines. For consistency also add a final new
line character to the printk call in start_aps.

We only ever start and execute threads on the BSP. By explicitly
checking to see if the CPU is the BSP we can remove the dependency on
cpu_info. With this change we can in theory enable threads in all
stages.

There is currently a fundamental flaw in the current cpu_info()
implementation. It assumes that current stack is CONFIG_STACK_SIZE
aligned. This assumption breaks down when performing SMM relocation.
The first step in performing SMM relocation is changing the SMBASE. This
is accomplished by installing the smmstub at 0x00038000, which is the
default SMM entry point. The stub is configured to set up a new stack
with the size of 1 KiB (CONFIG_SMM_STUB_STACK_SIZE), and an entry point
of smm_do_relocation located in RAMSTAGE RAM.
This means that when smm_do_relocation is executed, it is running in SMM
with a different sized stack. When cpu_info() gets called it will be
using CONFIG_STACK_SIZE to calculate the location of the cpu_info
struct. This results in reading random memory. Since cpu_info() has to
run in multiple environments, we can't use a compile time constant to
locate the cpu_info struct.
This CL introduces a new way of locating cpu_info. It uses a per-cpu
segment descriptor that points to a per-cpu segment that is allocated on
the stack. By using a segment descriptor to point to the per-cpu data,
we no longer need to calculate the location of the cpu_info struct. This
has the following advantages:
* Stacks no longer need to be CONFIG_STACK_SIZE aligned.
* Accessing an unconfigured segment will result in an exception. This
ensures no one can call cpu_info() from an unsupported environment.
* Segment selectors are cleared when entering SMM and restored when
leaving SMM.
* There is a 1:1 mapping between cpu and cpu_info. When using
COOP_MULTITASKING, a new cpu_info is currently allocated at the top of
each thread's stack. This no longer needs to happen.
This CL guards most of the code with CONFIG(CPU_INFO_V2). I did this so
reviewers can feel more comfortable knowing most of the CL is a no-op. I
would eventually like to remove most of the guards though.
This CL does not touch the LEGACY_SMP_INIT code path. I don't have any
way of testing it.
The %gs segment was chosen over the %fs segment because it's what the
linux kernel uses for per-cpu data in x86_64 mode.

Since current AMD SoCs don't need some wait time between INIT and SIPI,
we can skip the 10ms wait there, which improves the boot time a bit.
before: CPU_CLUSTER: 0 init finished in 632 msecs
after: CPU_CLUSTER: 0 init finished in 619 msecs
mpinit still works on Mandolin and all CPU cores show up and are usable.
This also doesn't change the binary in a timeless build for boards/SoCs
that don't select X86_AMD_INIT_SIPI which I verified for lenovo/x230.

The alignment for `struct cpu_info` is wrong on x86_64. c_start.S uses
the `push` instruction when setting up the cpu_info struct. This
instruction will push 8 bytes but `unsigned int` is 4 bytes. By making
it a `size_t` we get the correct size for both x86_32 and x86_64.

Trigger mode LAPIC_INT_LEVELTRIG was only used with LAPIC_DM_INIT,
specifically for (obsolete) Init Level De-assert.
Level LAPIC_INT_ASSERT is required to be set for all other delivery
modes other than LAPIC_DM_INIT.
This reverts the two above changes that X2APIC mode support introduced
to the IPI for LAPIC_DM_SMI.

Use the same stack location during relocation as for the permanent
handler.
When the number of CPUs is too large the stacks during relocation
don't fit inside the default SMRAM segment at 0x30000. Currently the
code would just let the CPU stack base grow downwards outside of the
default SMM segment which would corrupt lower memory if S3 is
implemented.
Also update the comment on smm_module_setup_stub().

Implement x2apic mode as existing code only supports apic mode.
Use info from LAPIC_BASE_MSR (LAPIC_BASE_MSR_X2APIC_MODE) to check
if apic mode or x2apic mode and implement x2apic mode according to
x2apic specfication.

Global variables are located in .bss and not on the CPU stack.
Overwriting them a per CPU case is bound to cause race conditions. In
this case it is even just plainly wrong.
Note: This variable is set up in the get_smm_info() function.

The permanent handler module argument 'save_state_size' now holds the
meaning of the real save state size which is then substracted from the
CPUs save state 'top' to get the save state base.
TESTED with qemu Q35 on x86_64 where the stub size exceeds the AMD64
save state size.

The parameters that the permanent handler requires are pushed directly
to the permanent handlers relocatable module params.
The paremeters that the relocation handler requires are not passed on
via arguments but are copied inside the ramstage. This is ok as the
relocation handler calls into ramstage.

The data needed to compute the permanent smbase for a core, when
relocating, is present in the ramstage data which the stub located at
DEFAULT_SMBASE (0x30000) calls back to. There is no need to fetch this
from via the stub params.

Increase timeout for CPUs to check in after 2nd SIPI completion
from 10ms to 100ms.
Update logging level for mp init failure cases from BIOS_DEBUG
to BIOS_ERR.
Without this patch, "mp initialization failure" happens on some
reboots on DeltaLake server. As consequence, not all 52 cpus
come up in Linux:

Xeon-SP Skylake Scalable Processor can have 36 CPU threads (18 cores).
Current coreboot SMM is unable to handle more than ~32 CPU threads.
This patch introduces a version 2 of the SMM module loader which
addresses this problem. Having two versions of the SMM module loader
prevents any issues to current projects. Future Xeon-SP products will
be using this version of the SMM loader. Subsequent patches will
enable board specific functionality for Xeon-SP.
The reason for moving to version 2 is the state save area begins to
encroach upon the SMI handling code when more than 32 CPU threads are
in the system. This can cause system hangs, reboots, etc. The second
change is related to staggered entry points with simple near jumps. In
the current loader, near jumps will not work because the CPU is jumping
within the same code segment. In version 2, "far" address jumps are
necessary therefore protected mode must be enabled first. The SMM
layout and how the CPUs are staggered are documented in the code.
By making the modifications above, this allows the smm module loader to
expand easily as more CPU threads are added.
TEST=build for Tiogapass platform under OCP mainboard. Enable the
following in Kconfig.
select CPU_INTEL_COMMON_SMM
select SOC_INTEL_COMMON_BLOCK_SMM
select SMM_TSEG
select HAVE_SMI_HANDLER
select ACPI_INTEL_HARDWARE_SLEEP_VALUES
Debug console will show all 36 cores relocated. Further tested by
generating SMI's to port 0xb2 using XDP/ITP HW debugger and ensured all
cores entering and exiting SMM properly. In addition, booted to Linux
5.4 kernel and observed no issues during mp init.

This update is a combination of all four of the patches so that the
commit can be done without breaking parts of coreboot. This possible
breakage is because of the cross-dependencies between the original
separate patches would cause failure because of data structure changes.
security/intel/stm
This directory contains the functions that check and move the STM to the
MSEG, create its page tables, and create the BIOS resource list.
The STM page tables is a six page region located in the MSEG and are
pointed to by the CR3 Offset field in the MSEG header. The initial
page tables will identity map all memory between 0-4G. The STM starts
in IA32e mode, which requires page tables to exist at startup.
The BIOS resource list defines the resources that the SMI Handler is
allowed to access. This includes the SMM memory area where the SMI
handler resides and other resources such as I/O devices. The STM uses
the BIOS resource list to restrict the SMI handler's accesses.
The BIOS resource list is currently located in the same area as the
SMI handler. This location is shown in the comment section before
smm_load_module in smm_module_loader.c
Note: The files within security/intel/stm come directly from their
Tianocore counterparts. Unnecessary code has been removed and the
remaining code has been converted to meet coreboot coding requirements.

The current MP init timeout is hardcoded as 1s. To support
platform with many cpus, the timeout needs to be adjusted.
The number of cpus is calculated as:
number of sockets * number of cores per socket *
number of threads per core
How long the timeout should be set to, is heuristic.
It needs to be set long enough to ensure reboot stability,
but not unreasonable so that real failures can be detected
soon enough, especially for smaller systems.
This patch sets timeout to be minimum as 1 second, while each
cpu adds 0.1 second.

Some timeouts given were too small when serial console is enabled due to
its spinlock making code runtime worse with every AP present.
In addition we usually don't know how long specific code runs and how
long ago it was sent to the APs.
Remove the timeout argument from mp_run_on_all_cpus and instead wait up
to 1 second, to prevent possible crashing of secondary APs still
processing the old job.
Tested on Supermicro X11SSH-TF.

The common code adheres to the Intel requirement of bringing up the
cores with INIT SIPI SIPI. This sequence is tolerated on some AMD
AMD CPUs but fails on others. Add a way to skip the second SIPI.

Previously cpu_index() always succeeded, but since commit 095c931
(src/arch/x86: Use core apic id to get cpu_index()) it is now possible
for it to indicate an error by returning -1. This commit adds error
handling for all calls to cpu_index(), and restores several checks that
were removed in commit 7c712bb (Fix code that would trip -Wtype-limits)
but are now needed.

This patch renames mp_get_apic_id() to cpu_get_apic_id() and
add_cpu_map_entry() to cpu_add_map_entry() in order access it
outside CONFIG_PARALLEL_MP kconfig scope.

- Make cpu_add_map_entry() function available externally to call

When rmodule is loaded CPU stack alignment is only guaranteed
to 4kiB. Implementation of cpu_info() requires that each
CPU sees its stack aligned to CONFIG_STACK_SIZE.
Add one spare CPU for the stack reserve, such that alignment can
be enforced runtime.

A stack overrun has been observed on AMD64 CPUs during the SMM module
relocation process. Change the assumed required size from equaling the
save structure's size to a Kconfig symbol.
A value of 0x400 doubles the size used by AMD64 systems and maintains
the size used by EM64T.

Its spreading copies got out of sync. And as it is not a standard header
but used in commonlib code, it belongs into commonlib. While we are at
it, always include it via GCC's `-include` switch.
Some Windows and BSD quirk handling went into the util copies. We always
guard from redefinitions now to prevent further issues.

Within procedure save_bsp_msrs, the structure pointer "msr_entry" is updated
every time procedure save_msr() is called. However, after the last call of
save_msr(), "msr_entry" is not used, thus causing a static analysis error.
Add a "(void)msr_entry;" at the end to avoid the static analysis error.

In procedure allocate_cpu_devices(), if structure pointer new is null skip
using the pointer. Add a "continue;" to skip using the pointer.
The issue was found by static analysis tool.

In order to extend the MP callback infrastructure prepare for
easier changes by making the AP callback get signalled by a
single pointer to a local variable on the signaller's stack.
When the APs see the callback they will copy the structure
to a local variable and then set the acknowledgement by
clearing out the slot.
The reading and writing to the slots were implemented using inline
assembly which forces a memory access and a compiler barrier.

barrier_wait_timeout() was not used anywhere in the code. The
remaining two functions, barrier_wait() and release_barrier(), are
not used anywhere but the mp code.

There might be certain requirement in user function where user
might not want to pass any timeout value, in those cases
run_ap_work() should consider infinity as timeout and perform
all APs initialization as per specification.
Set expire_us <= 0 to specify an infinite timeout.

Change f43adf0 (intel/common/block/cpu: Change post_cpus_init after
BS_DEV RESOURCES) moved post_cpus_init to BS_OS_RESUME for S3
path. This results in BSP timing out waiting for APs to be
parked. This change increases the time out value for APs to be parked
to 250ms. This value was chosen after running suspend-resume stress
test and capturing the maximum time taken for APs to be parked for
100 iterations. Typical values observed were ~150ms. Maximum value
observed was 152ms.

TEST=Verified for 100 iterations that suspend-resume does not run into
any AP park time out.

Since the timeout in bsp_do_flight_plan is bumped up to 1 second, this
change adds a print to indicate the amount of time it takes for all the
APs to check-in.
TEST=Verified on Nami that it prints:
"bsp_do_flight_plan done after 395 msecs."

Currently, the AP check-in time-out in bsp_do_flight_plan is set to
100ms. However, as the number of APs increases, contention could
increase especially for resource like UART. This led to MP record
time-out issues on KBL platform with 7 APs and serial-console enabled
BIOS image.
This change increases the time-out value to 1 second to be on the safer
side and let APs check-in before continuing boot.

AMD's fixed MTRRs have RdDram and WrDram bits that route memory
accesses to DRAM vs. MMIO. These are typically hidden for normal
operation by clearing SYS_CFG[19] (MtrrFixDramModEn). According to
BKDGs and AMD64 Programmer's Manual vol 2, this bit is clear at
reset, should be set for configuration during POST, then cleared for
normal operation.
Attempting to modify the RdDram and WrDram settings without unhiding
them causes a General Protection Fault. Add functions to enable and
disable MtrrFixDramModEn. Unhide/hide as necessary when copying or
writing the fixed MTRRs.
Finally, modify sipi_vector.S to enable the bits prior to writing
the fixed MTRRs and disable when complete.
This functionality is compiled out on non-AMD platforms.

Not all SMM save state sizes equate to having enough stack in the
permanent SMM handler. Therefore, ensure 1KiB of stack is available for
each cpu's stack. Intel's save state size is 1KiB, but AMD's save state
size is only 512. Therefore, decouple save state size from the per
cpu stack size.

The original purpose of adjust_cpu_apic_entry() was to set
up an APIC map. That map was effectively only used for mapping
*default* APIC id to CPU number in the SMM handler. The normal
AP startup path didn't need this mapping because it was whoever
won the race got the next cpu number. Instead of statically
calculating (and wrong) just initialize the default APIC id
map when the APs come online. Once the APs are online the SMM
handler is loaded and the mapping is utilized.

The previous implementation was using a for loop. By it's
very definition the last statement in the for loop declaration
is done at the end of the loop. Therefore, if the conditional for
breaking out of the for loop because of a timeout would always
see a value of 0 for the number of APs accepted. Correct this
by changing to a while loop with an explicit timeout condition
at the end of the loop.

In case something goes wrong on one of the
cpus, add the ability to use a barrier with
timeout so that other cpus don't wait forever.

There are circumstances where the APs need to run a piece of
code later in the boot flow. The current MP init just parks
the APs after MP init is completed so there's not an opportunity
to target running a piece of code on all the APs at a later time.
Therefore, provide an option, PARALLEL_MP_AP_WORK, that allows
the APs to perform callbacks.

In order to reduce code duplication provide a common flow
through callback functions that performs the multiprocessor
and optionally SMM initialization. The existing MP flight
records are utilized but a common flow is provided such
that the chipset/cpu only needs to provide a mp_ops
structure which has callbacks to gather info and provide
hooks at certain points in the sequence.
All current users of the MP code can be switched over to
this flow since there haven't been any flight records that
are overly complicated and long. After the conversion
has taken place most of the surface area of the MP
API can be hidden away within the compilation unit proper.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.
