Add Ramaxel DRAM manufacturer id.
Tested on Lenovo T520 and DDR3-1600 DIMM (RMT3170eb86e9w16).
The manufacturer name shows up in dmidecode.

Support ROM_SIZE greater than 16 MiB. Work around SMBIOS rom size
limitation of 16 MiB by specifying 16 MiB as the ROM size.

Different DIMM modules give different SMBIOS type 17 lengths, so we
can't use `meminfo->dimm_cnt*len' for entry struct size, otherwise
it'll give a wrong SMBIOS size when two or more different DIMMs are
installed on the machine.

Section 6.1.3 (Text Strings) of the SMBIOS specification states:
If a string field references no string, a null (0) is placed in that
string field.
Change smbios_add_string() to do that.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

It never made sense to have bootblock_* in init, but
pirq_routing.c in boot, and some ld scripts on the main
level while others live in subdirectories.
This patch flattens the directory hierarchy and makes
x86 more similar to the other architectures.
