Use uintptr_t for the IOAPIC base parameter of the various IOAPIC-
related functions to avoid needing type casts in the callers. This also
allows dropping the VIO_APIC_VADDR define and consistently use the
IO_APIC_ADDR define instead.

Back in the days of the APIC bus, the IOAPIC IDs mustn't overlap with
the LAPIC IDs (0 to CONFIG_MAX_CPUS - 1), but since the IOAPIC and LAPIC
nowadays talk to each other via the system bus, an IOAPIC ID of 0 is
valid. When set_ioapic_id gets called with an IOAPIC ID of 0, it skipped
writing the IOAPIC ID to the corresponding IOAPIC register, so the code
was relying of the register having the expected default value of the
IOAPIC IO 0 for things to work as expected. The case of the IOAPIC ID
being 0 is the most common case in coreboot, since that's what
register_new_ioapic_gsi0 will end up doing. Fix this issue by not making
the io_apic_write call conditional on ioapic_id being non-zero. The only
southbridge that doesn't call register_new_ioapic_gsi0, calls
set_ioapic_id with the IOAPIC ID 2 for which this won't cause any
changes in behavior.

Using this I/O APIC IDs will be assigned incrementally
in the order of calling. I/O APIC ID #0 is reserved for
the I/O APIC delivering GSI #0.

The commit 04a40379b has a wrongly written variable, which sets an
IOAPIC register to a wrong value and makes the Linux kernel unable to
boot.
Tested on HP EliteBook 2760p, the kernel boots after this patch.

The number of redirection table entries (aka interrupt vectors) inside
an I/O APIC may depend of the SKU, with the related register being of
type read/write-once. Provide support utilities to either lock or set
this registers value.

All boards with DRIVERS_GENERIC_IOAPIC select it.
Presumably the related configuration of routing IRQ0 when
IOAPIC is enabled should be always done to provide i8259
legacy compatibility for payloads.

APIC Serial Bus pins were removed with ICH5 already, so a choice
'irq_on_fsb = 0' would not take effect. The related register BOOT_CONFIG
0x3 is also not documented since ICH5.
For emulation/qemu-q35 with ICH9 the choice INTERRUPT_ON_APIC_BUS was
wrong and ignored as BOOT_CONFIG register emulation was never implemented.
For ICH4 and earlier, the choice to use FSB can be made based on the
installed CPU model but this is now just hardwired to match P4 CPUs of
aopen/dxplplusu.
For sb/intel/i82371eb register BOOT_CONFIG 0x3 is also not defined
and the only possible operation mode there is APIC Serial Bus, which
requires no configuration.

According to the POSIX standard, %p is supposed to print a pointer "as
if by %#x", meaning the "0x" prefix should automatically be prepended.
All other implementations out there (glibc, Linux, even libpayload) do
this, so we should make coreboot match. This patch changes vtxprintf()
accordingly and removes any explicit instances of "0x%p" from existing
format strings.
How to handle zero padding is less clear: the official POSIX definition
above technically says there should be no automatic zero padding, but in
practice most other implementations seem to do it and I assume most
programmers would prefer it. The way chosen here is to always zero-pad
to 32 bits, even on a 64-bit system. The rationale for this is that even
on 64-bit systems, coreboot always avoids using any memory above 4GB for
itself, so in practice all pointers should fit in that range and padding
everything to 64 bits would just hurt readability. Padding it this way
also helps pointers that do exceed 4GB (e.g. prints from MMU config on
some arm64 systems) stand out better from the others.

is going to move the majority of copyrights out of the headers and into
an AUTHORS file. This will happen a bit at a time, as we'll be unifying
license headers at the same time.

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

It never made sense to have bootblock_* in init, but
pirq_routing.c in boot, and some ld scripts on the main
level while others live in subdirectories.
This patch flattens the directory hierarchy and makes
x86 more similar to the other architectures.
