It's useful to see the stack when an exception happens so you can see
the variables on the stack, and also manually recreate the back trace.
If you need to recreate the back trace, you will need to add
-fno-omit-frame-pointer to the CFLAGS.

TEST=Caused an exception and saw the stack dumped. Then I manually
recreated the back trace.

Previously cpu_index() always succeeded, but since commit 095c931
(src/arch/x86: Use core apic id to get cpu_index()) it is now possible
for it to indicate an error by returning -1. This commit adds error
handling for all calls to cpu_index(), and restores several checks that
were removed in commit 7c712bb (Fix code that would trip -Wtype-limits)
but are now needed.

Its spreading copies got out of sync. And as it is not a standard header
but used in commonlib code, it belongs into commonlib. While we are at
it, always include it via GCC's `-include` switch.
Some Windows and BSD quirk handling went into the util copies. We always
guard from redefinitions now to prevent further issues.

Add Kconfig IDT_IN_EVERY_STAGE to optionally specify having
the interrupt handling code available to all stages. In order
to do this the idt setup is moved to a C module. The vecX
entries are made global so that a table of references to all
the interrupt vector entry points can be used to dynamically
initialize the idt. The ramification for ramstage is that
exceptions are initialized later (lib/hardwaremain.c). Not
all stages initialize exceptions when this Kconfig variable
is selected, but bootblock for the C, stages using
assembly_entry.S, and of course ramstage do. Anything left
out just needs a call to exception_init() at the right
location.

These were mostly written as part of the coreboot project, so get
the standard coreboot license header.
memmove.c came from the linux kernel, so also gets the standard
coreboot v2 license header, but gets the added attribution that it
was derived from the linux kernel. Unlike many coreboot files,
this file may not be re-licensed as GPL V3.

It never made sense to have bootblock_* in init, but
pirq_routing.c in boot, and some ld scripts on the main
level while others live in subdirectories.
This patch flattens the directory hierarchy and makes
x86 more similar to the other architectures.
