Add support for devices with the reset vector pointing into DRAM. This
is a specific implementation that assumes a paradigm of AMD Family 17h
(a.k.a. "Zen"). Until the first ljmpl for protected mode, the core's
state appears to software like other designs, and then the actual
physical addressing becomes recognizable.
These systems cannot implement cache-as-RAM as in more traditional
x86 products. Therefore instead of reusing CAR names and variables,
a substitute called "earlyram" is introduced. This change makes
adjustments to CAR-aware files accordingly.
Enable NO_XIP_EARLY_STAGES. The first stage is already in DRAM, and
running subsequent stages as XIP in the boot device would reduce
performance.
Finally, add a new early_ram.ld linker file. Because all stages run in
DRAM, they can be linked with their .data and .bss as normal, i.e. they
don't need to rely on storage available only at a fixed location like
CAR systems. The primary purpose of the early_ram.ld is to provide
consistent locations for PRERAM_CBMEM_CONSOLE, TIMESTAMP regions, etc.
across stages until cbmem is brought online.

When CONFIG_SEPARATE_VERSTAGE=n, all verstage code gets linked into the
appropriate calling stage (bootblock or romstage). This means that
ENV_VERSTAGE is actually 0, and instead ENV_BOOTBLOCK or ENV_ROMSTAGE
are 1. This keeps tripping up people who are just trying to write a
simple "are we in verstage (i.e. wherever the vboot init logic runs)"
check, e.g. for TPM init functions which may run in "verstage" or
ramstage depending on whether vboot is enabled. Those checks will not
work as intended for CONFIG_SEPARATE_VERSTAGE=n.
This patch renames ENV_VERSTAGE to ENV_SEPARATE_VERSTAGE to try to
clarify that this macro can really only be used to check whether code is
running in a *separate* verstage, and clue people in that they may need
to cover the linked-in verstage case as well.

The romcc bootblock will be deprecated soon and most platforms use
C_ENVIRONMENT_BOOTBLOCK already. This patch drops the
CONFIG_C_ENVIRONMENT_BOOTBLOCK symbol and adds CONFIG_ROMCC_BOOTBLOCK
where needed.

Restrict the use of symbol names _car_global_[start|end]
to be used exclusively with CAR_GLOBAL_MIGRATION=y.
They just alias the start and end of .bss section in CAR.

As discussed on the mailing list and voted upon, the coreboot project
is going to move the majority of copyrights out of the headers and into
an AUTHORS file. This will happen a bit at a time, as we'll be unifying
license headers at the same time.

Many (if not all) platforms have moved to using consistent
boot flow constructs where a weak car_stage_entry() is no longer
necessary to avoid the complexity of handling the numerous and
different boot flow combinations. The weak symbol is just causing
issues so remove it.

In each stage keep GDT in the code region. This accommodates
platforms, such as glk, that are executing out of CAR. The
gdt is small and loading it is trivial so just do it unconditionally
instead of introducing another Kconfig.

Now that VBOOT_STARTS_IN_BOOTBLOCK depends on C_ENVIRONMENT_BOOTBLOCK,
remove the complications in assembly_entry.S. There's no platform
utilizing romcc bootblock and needing to handle verified boot after
bootblock as well as not using verified boot. That combination makes
things very complicated. Clean up the complication as it's not a
combination that needs to be supported.

Add Kconfig IDT_IN_EVERY_STAGE to optionally specify having
the interrupt handling code available to all stages. In order
to do this the idt setup is moved to a C module. The vecX
entries are made global so that a table of references to all
the interrupt vector entry points can be used to dynamically
initialize the idt. The ramification for ramstage is that
exceptions are initialized later (lib/hardwaremain.c). Not
all stages initialize exceptions when this Kconfig variable
is selected, but bootblock for the C, stages using
assembly_entry.S, and of course ramstage do. Anything left
out just needs a call to exception_init() at the right
location.

At process _start, the stack is expected to be aligned to a
16-byte boundary. Upon entry to any function the stack frame
must have the end of any arguments also aligned. In other words
the value of %esp+4 or %rsp+8 is always a multiple of 16 (1).
Align the stack down and change the method for executing
car_stage_entry from jmp to call which should preserve proper
alignment regardless of a 32- or 64-bit build.
Although 4-byte alignment is the minimum requirement for i386,
some AMD platforms use SSE instructions which expect 16-byte.

This patch attempts to finish the separation between CONFIG_VBOOT and
CONFIG_CHROMEOS by moving the remaining options and code (including
image generation code for things like FWID and GBB flags, which are
intrinsic to vboot itself) from src/vendorcode/google/chromeos to
src/vboot. Also taking this opportunity to namespace all VBOOT Kconfig
options, and clean up menuconfig visibility for them (i.e. some options
were visible even though they were tied to the hardware while others
were invisible even though it might make sense to change them).

The skylake-based Chromebooks use a separate verstage which runs
just after bootblock and prior to romstage. The normal path for
romstage would be to reload the gdt, however in the previously
described scenario has verstage performing that work. Therefore,
provide that path under those conditions. The only difference
from the C_ENVIRONMENT_BOOTBLOCK scenario is that the stack
should not be reloaded since there's no way to know the top
of the stack.

The path that just clears CAR_GLOBAL variables and jumps
to the stage entry point needs another condition for
separate verstage just after bootblock. However, the
current conditional is a negative conditional so
swap the logic around to make it easier to extend.

This adds a few assembly lines that are generic enough to be shared
between romstage and verstage that are ran in CAR. The GDT reload
is bypassed and the stack is reloaded with the CAR stack defined
in car.ld. The entry point for all those stages is car_stage_entry().

It encourages users from writing to the FSF without giving an address.
Linux also prefers to drop that and their checkpatch.pl (that we
imported) looks out for that.
This is the result of util/scripts/no-fsf-addresses.sh with no further
editing.

In order to support verstage on x86 one needs to link verstage
like romstage since it needs all the cache-as-ram goodies. Therefore,
provide a macro that one can invoke that provides the necessary
recipes for linking that particular stage in such an environment.
