This solution is very generic and can in principle be implemented on
all arch/soc. Currently the old infrastructure to pass on information
from romstage to ramstage is left in place and will be removed in a
follow-up commit.
Nvidia Tegra will be handled in a separate patch because it has a
custom ramstage entry.
Instead trying to figure out which files can be removed from stages
and which cbmem_top implementations need with preprocessor, rename all
cbmem_top implementation to cbmem_top_romstage.
Mechanisms set in place to pass on information from rom- to ram-stage
will be replaced in a followup commit.

This patch renames arm_tf.c and arm_tf.h to bl31.c and bl31.h,
respectively. That name is closer to the terminology used in most
functions related to Trusted Firmware, and it removes the annoying
auto-completion clash between arm64/arm_tf.c and arm64/armv8.

As discussed on the mailing list and voted upon, the coreboot project
is going to move the majority of copyrights out of the headers and into
an AUTHORS file. This will happen a bit at a time, as we'll be unifying
license headers at the same time.
Additional changes in this patch:
- Make sure files say that they're part of the coreboot project
- Move descriptions below the license header
Note that the file include/arch/acpi.h is a fantastic example of why
moving to the authors file is needed. Excluding the guard statements,
it has 8 lines of copyrights for 3 function declarations.

Bounce buffers used to be used in those cases where the payload
might overlap coreboot.
Bounce buffers are a problem for rampayloads as they need malloc.
They are also an artifact of our x86 past before we had relocatable
ramstage; only x86, out of the 5 architectures we support, needs them;
currently they only seem to matter on the following chipsets:
src/northbridge/amd/amdfam10/Kconfig
src/northbridge/amd/lx/Kconfig
src/northbridge/via/vx900/Kconfig
src/soc/intel/fsp_baytrail/Kconfig
src/soc/intel/fsp_broadwell_de/Kconfig
The first three are obsolete or at least could be changed
to avoid the need to have bounce buffers.
The last two should change to no longer need them.
In any event they can be fixed or pegged to a release which supports
them.
For these five chipsets we change CONFIG_RAMBASE from 0x100000 (the
value needed in 1999 for the 32-bit Linux kernel, the original ramstage)
to 0xe00000 (14 Mib) which will put the non-relocatable x86
ramstage out of the way of any reasonable payload until we can
get rid of it for good.
14 MiB was chosen after some discussion, but it does fit well:
o Fits in the 16 MiB cacheable range coreboot sets up by default
o Most small payloads are well under 14 MiB (even kernels!)
o Most large payloads get loaded at 16 MiB (especially kernels!)
With this change in place coreboot correctly still loads a bzImage payload.
Werner reports that the 0xe00000 setting works on his broadwell systems.

Its spreading copies got out of sync. And as it is not a standard header
but used in commonlib code, it belongs into commonlib. While we are at
it, always include it via GCC's `-include` switch.
Some Windows and BSD quirk handling went into the util copies. We always
guard from redefinitions now to prevent further issues.

When we first created the arm64 port, we weren't quite sure whether
coreboot would always run in EL3 on all platforms. The AArch64 A.R.M.
technically considers this exception level optional, but in practice all
SoCs seem to support it. We have since accumulated a lot of code that
already hardcodes an implicit or explicit assumption of executing in EL3
somewhere, so coreboot wouldn't work on a system that tries to enter it
in EL1/2 right now anyway.
However, some of our low level support libraries (in particular those
for accessing architectural registers) still have provisions for
running at different exception levels built-in, and often use switch
statements over the current exception level to decide which register to
access. This includes an unnecessarily large amount of code for what
should be single-instruction operations and precludes further
optimization via inlining.
This patch removes any remaining code that dynamically depends on the
current exception level and makes the assumption that coreboot executes
at EL3 official. If this ever needs to change for a future platform, it
would probably be cleaner to set the expected exception level in a
Kconfig rather than always probing it at runtime.

cache_sync_instructions() has been superseded by
arch_program_segment_loaded() and friends for a while. There are no uses
in common code anymore, so let's remove it from <arch/cache.h> for all
architectures.
arm64 still has an implementation and one reference, but they are not
really needed since arch_program_segment_loaded() does the same thing
already. Remove them.
Leave it in arm(32) since there are several references (including in SoC
code) that I don't feel like tracking down and testing right now.

The PSTATE mask bits for Debug exceptions, external Aborts, Interrupts
and Fast interrupts are usually best left unset: under normal
circumstances none of those exceptions should occur in firmware, and if
they do it's better to get a crash close to the code that caused it
(rather than much later when the kernel first unmasks them). For this
reason arm64_cpu_init unmasks them right after boot. However, the EL2
payload was still running with all mask bits set, which this patch
fixes.
BL31, on the other hand, explicitly wants to be entered with all masks
set (see calling convention in docs/firmware-design.md), which we had
previously not been doing. It doesn't seem to make a difference at the
moment, but since it's explicitly specified we should probably comply.

The existing arm64 architecture code has been developed for the Tegra132
and Tegra210 SoCs, which only start their ARM64 cores in ramstage. It
interweaves the stage entry point with code that initializes a CPU (and
should not be run again if that CPU already ran a previous stage). It
also still contains some vestiges of SMP/secmon support (such as setting
up stacks in the BSS instead of using the stage-peristent one from
memlayout).
This patch splits those functions apart and makes the code layout
similar to how things work on ARM32. The default stage_entry() symbol is
a no-op wrapper that just calls main() for the current stage, for the
normal case where a stage ran on the same core as the last one. It can
be overridden by SoC code to support special cases like Tegra.
The CPU initialization code is split out into armv8/cpu.S (similar to
what arm_init_caches() does for ARM32) and called by the default
bootblock entry code. SoCs where a CPU starts up in a later stage can
call the same code from a stage_entry() override instead.
The Tegra132 and Tegra210 code is not touched by this patch to make it
easier to review and validate. A follow-up patch will bring those SoCs
in line with the model.

As per discussion with lawyers[tm], it's not a good idea to
shorten the license header too much - not for legal reasons
but because there are tools that look for them, and giving
them a standard pattern simplifies things.
However, we got confirmation that we don't have to update
every file ever added to coreboot whenever the FSF gets a
new lease, but can drop the address instead.
util/kconfig is excluded because that's imported code that
we may want to synchronize every now and then.

The configuration of SPSR bits that mask processor exceptions is kinda
oddly hidden as an implict part of the transition() function right now.
It would be odd but not impossible for programs to want to be entered
with enabled exceptions, so let's move these bits to be explicitly set
by the caller like the rest of SPSR instead.
Also clear up some macro names. The SPSR[I] bit is currently defined as
SPSR_IRQ_ENABLE, which is particularly unfortunate since that bit
actually *disables* (masks) interrupts. The fact that there is an
additional SPSR_IRQ_MASK definition with the same value but a different
purpose doesn't really help. There's rarely a point to have all three of
xxx_SHIFT, xxx_MASK and xxx_VALUE macros for single-bit fields, so
simplify this to a single definition per bit. (Other macros in
lib_helpers.h should probably also be overhauled to conform, but I want
to wait and see how many of them really stay relevant after upcoming
changes first.)

This patch slightly reorganizes arm64/boot.c with the aim of being more
readable: the secure monitor entry is now guarded by an explicit if
statement for its Kconfig rather than hiding than in the corresponding
header file. This makes it clear that there are two (soon three)
separate code paths here.

Fix up commit b3847e64 (program loading: add prog_run() function),
which misses the braces for the if statement, causing the function
also to return if a non-payload program should be run causing the rest
of the stages never to be run.

ramstage and payload align to using struct prog.
This also removes arch_payload_run() in favor of

The prog_run() function abstracts away what is required
for running a given program. Within it, there are 2
calls: 1. platform_prog_run() and 2. arch_prog_run().
The platform_prog_run() allows for a chipset to intercept
a program that will be run. This allows for CPU switching
as currently needed in t124 and t132.

The struct prog serves as way to consolidate program
loading. This abstraction can be used to perform more
complicated execution paths such as running a program
on a separate CPU after it has been loaded. Currently
t124 and t132 need to do that in the boot path. Follow
on patches will allow the platform to decide how to
execute a particular program.
Note: the vboot path is largely untouched because it's
already broken in the coreboot.org tree. After getting
all the necessary patches pushed then vboot will be
fixed.

Secure monitor runs at EL3 and is responsible for jumping to the payload at
specified EL and also to manage features like PSCI.
Adding basic implementation of secure monitor as a rmodule. Currently, it just
jumps to the the payload at current EL. Support for switching el and PSCI will
be added as separate patches.

patch ports the following changes from ARMV4/V7 to ARMV8:
- make armv8 Kconfig options consistent with armv4/v7
- fix build include issues in boot.c, tables.c,
and early_variables.h by matching armv4/v7.

these patches:
d955885 coreboot: Rename coreboot_ram stage to ramstage
a492761 cbmem console: Locate the preram console with a symbol instead of a sect
96e7f0e aarch64: Enable early icache and migrate SCTLR from EL3
3f854dc aarch64: Pass coreboot table in jmp_to_elf_entry
ab3ecaf aarch64/foundation-armv8: Set up RAM area and enter ramstage
25fd2e9 aarch64: Remove CAR definitions from early_variables.h
65bf77d aarch64/foundation-armv8: Enable DYNAMIC_CBMEM

c732a9d aarch64/foundation-armv8: Basic bootblock implementation
3bc412c aarch64: Comment out some parts of code to allow build
ab5be71 Add initial aarch64 support
The ramstage support is the only portion that has been tested
on actual hardware. Bootblock and romstage support may require
modifications to run on hardware.
